摘要:
目的 评价生长抑素联合艾司奥美拉唑对上消化道出血患者凝血指标及血清炎性因子的影响。方法 选取2020年6月-2024年6月万安县中医院就诊的100例急性上消化道出血患者作为研究对象，按照随机数字表法将其分为对照组和研究组，每组50例。对照组给予艾司奥美拉唑治疗，研究组在对照组基础上给予注射用生长抑素治疗。比较两组临床治疗效果、凝血功能指标（凝血酶原时间、活化部分凝血活酶时间、血小板计数）、炎性因子指标水平[C反应蛋白（CRP）、白细胞计数（WBC）]、不良反应发生率（胸闷、腹痛、头晕、胃肠道感染、再出血）。结果 研究组治疗总有效率（96.00%）高于对照组（72.00%）（P＜0.05）；研究组治疗后凝血酶原时间、活化部分凝血活酶时间低于对照组，而血小板计数高于对照组（P＜0.05）；研究组治疗后CRP、WBC水平低于对照组（P＜0.05）；两组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论 生长抑素联合艾司奥美拉唑治疗上消化道出血患者的临床疗效确切，能够改善患者凝血功能，降低血清炎性因子水平，且安全性较高。
Abstract:
Objective To evaluate the effect of somatostatin combined with esomeprazole on coagulation indexes and serum inflammatory factors in patients with upper gastrointestinal bleeding.Methods A total of 100 patients with acute upper gastrointestinal bleeding who were treated in Wan′an County Hospital of Traditional Chinese Medicine from June 2020 to June 2024 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 50 patients in each group. The control group was treated with esomeprazole, and the study group was treated with somatostatin for injection on the basis of the control group. The clinical treatment effect, coagulation function indexes (prothrombin time, activated partial thromboplastin time, platelet count), inflammatory factor index levels [C-reactive protein (CRP), white blood cell count (WBC)], and incidence of adverse reactions (chest tightness, abdominal pain, dizziness, gastrointestinal infection, rebleeding) were compared between the two groups.Results The total effective rate of treatment in the study group (96.00%) was higher than that in the control group (72.00%) (P<0.05). After treatment, the prothrombin time and activated partial thromboplastin time in the study group were lower than those in the control group, while the platelet count was higher than that in the control group (P<0.05). After treatment, the levels of CRP and WBC in the study group were lower than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Somatostatin combined with esomeprazole is effective in the treatment of patients with upper gastrointestinal bleeding, which can improve the coagulation function, reduce the level of serum inflammatory factors, and has high safety.
