摘要:
目的 分析超声及其引导下细针穿刺细胞学检查对乳腺筛查中触诊阴性结节的诊断价值。方法 选取2020年6月-2024年5月新余市渝水区妇幼保健院接受乳腺筛查中,触诊阴性结节的患者100例作为研究对象,所有研究对象均进行超声及其引导下细针穿刺细胞学检查诊断,同时将手术病理组织活检结果作为金标准,记录诊断结果,并且比较两种检查方法诊断触诊阴性乳腺结节良恶性的价值。结果 术后组织病理活检结果显示,100例患者中,良性病变20例,恶性病变80例;超声引导下细针穿刺细胞学检查结果显示,BI-RADS 4a类乳腺肿块病灶中74.07%为恶性肿块;BI-RADS 4b类乳腺肿块病灶中35.29%为恶性肿块;BI-RADS 4c类乳腺肿块病灶中96.43%为恶性肿块。超声引导下细针穿刺细胞学检测的特异度、敏感度、阳性预测值、阴性预测值及诊断准确率均高于常规超声(P<0.05)。结论 在触诊阴性乳腺结节的诊断中,通过超声及其引导下细针穿刺细胞学检查诊断,具有准确率高、灵敏度高及特异度高等特点,有助于临床早期诊断,能辅助医生观察病灶情况,具备较高的诊断价值,值得临床应用。
Abstract:
Objective To analyze the diagnostic value of ultrasound and ultrasound-guided fine needle aspiration cytology in palpation negative nodules in breast screening.Methods From June 2020 to May 2024, 100 patients with negative palpation nodules in breast screening in Xinyu Yushui District Maternal and Child Health Hospital were selected as the research objects. All the subjects were diagnosed by ultrasound and ultrasound-guided fine needle aspiration cytology. At the same time, the results of surgical pathological biopsy were taken as the gold standard, and the diagnostic results were recorded. The value of the two methods in the diagnosis of benign and malignant breast nodules with negative palpation was compared.Results Postoperative histopathological biopsy results showed that among the 100 patients, there were 20 patients with benign lesions and 80 patients with malignant lesions. The results of ultrasound-guided fine needle aspiration cytology showed that 74.07% of BI-RADS 4a breast masses were malignant masses, 35.29% of BI-RADS 4b breast masses were malignant masses, 96.43% of BI-RADS 4c breast masses were malignant masses, the specificity, sensitivity, positive predictive value, negative predictive value and diagnostic accuracy of ultrasound-guided fine needle aspiration cytology were higher than those of conventional ultrasound (P<0.05).Conclusion In the diagnosis of palpable negative breast nodules, the diagnosis of ultrasound and ultrasound-guided fine needle aspiration cytology has the characteristics of high accuracy, high sensitivity and high specificity. It is helpful for early clinical diagnosis and can assist doctors to observe the lesions. It has high diagnostic value and is worthy of clinical application.
