摘要:
目的 研究加味白术芍药汤治疗糖尿病胃肠功能紊乱的临床疗效及对其营养状况的影响。方法 选取2022年4月-2023年4月在我院诊治的64例糖尿病胃肠功能紊乱患者为研究对象，采用随机数字表法分为对照组和观察组，各组32例。对照组采用常规治疗，观察组在对照组基础上给予加味白术芍药汤治疗，比较两组临床疗效、4 h胃排空率、中医证候积分、胃功能指标[胃动素（MTL）、胃泌素（GAS）]、营养状况指标[白蛋白（ALB）、转铁蛋白（TRF）]、血糖指标[空腹血糖（FBG），餐后2 h血糖（2 h PBG）]。结果 观察组治疗总有效率为96.88%，高于对照组的84.38%（P＜0.05）；两组治疗后4 h胃排空率均大于治疗前，且观察组大于对照组（P＜0.05）；两组治疗后中医证候积分均低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后MTL均高于治疗前，GAS均低于治疗前，且观察组MTL高于对照组，GAS低于对照组（P＜0.05）；观察组治疗后ALB、TRF均高于治疗前，且观察组高于对照组（P＜0.05），但对照组治疗前后比较，差异无统计学意义（P＞0.05）；两组治疗后FBG、2 h PBG均低于治疗前，且观察组低于对照组（P＜0.05）。结论 加味白术芍药汤治疗糖尿病胃肠功能紊乱的临床疗效确切，可提高治疗总有效率，减轻患者临床症状，改善胃功能和营养状况，促进血糖指标降低，值得临床加以应用。
Abstract:
Objective To study the clinical efficacy of Jiawei Baizhu Shaoyao decoction in the treatment of diabetic gastrointestinal dysfunction and its effect on nutritional status. Methods A total of 64 patients with diabetic gastrointestinal dysfunction diagnosed and treated in our hospital from April 2022 to April 2023 were selected as the research objects. They were divided into control group and observation group by random number table method, with 32 patients in each group. The control group was treated with routine treatment, and the observation group was treated with modified Baizhu Shaoyao decoction on the basis of the control group. The clinical efficacy, 4 h gastric emptying rate, TCM syndrome score, gastric function index [motilin (MTL), gastrin (GAS)], nutritional status index [albumin (ALB), transferrin (TRF)], blood glucose index [fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h PBG)] were compared between the two groups. Results The total effective rate of treatment in the observation group was 96.88%, which was higher than 84.38% in the control group (P<0.05). The gastric emptying rate at 4 h after treatment in the two groups was higher than that before treatment, and that in the observation group was higher than that in the control group (P<0.05). After treatment, the score of TCM syndromes in the two groups was lower than that before treatment, and that in the observation group was lower than that in the control group (P<0.05). After treatment, MTL in the two groups was higher than that before treatment, GAS was lower than that before treatment, while MTL in the observation group was higher than that in the control group, GAS was lower than that in the control group (P<0.05). After treatment, ALB and TRF in the observation group were higher than those before treatment, and those in the observation group were higher than those in the control group (P<0.05), but there was no significant difference in the control group before and after treatment (P>0.05). After treatment, FBG and 2 h PBG in the two groups were lower than those before treatment, and those in the observation group were lower than the control group (P<0.05). Conclusion Jiawei Baizhu Shaoyao decoction has a definite clinical effect in the treatment of diabetic gastrointestinal dysfunction, which can improve the total effective rate of treatment, reduce the clinical symptoms of patients, improve gastric function and nutritional status, and promote the reduction of blood glucose index. It is worthy of clinical application.
