摘要:
目的 分析输尿管软镜钬激光碎石术治疗输尿管上段结石的效果。方法 采用随机数字表法将2023年1月-12月遂川县中医院收治的上尿路结石患者50例分为对照组和观察组，各25例。对照组采取输尿管硬镜碎石取石术治疗，观察组采取输尿管软镜下钬激光碎石术治疗。比较两组治疗效果、手术及康复指标（碎石时间、手术时间、住院时间、双J管留置时间）、并发症发生率、炎症因子水平[白细胞介素-6（IL-6）、C反应蛋白（CRP）]、结石清除效果。结果 研究组治疗总有效率为96.67%，高于对照组的73.33%（P＜0.05）；研究组碎石时间、手术时间、住院时间、双J管留置时间均短于对照组（P＜0.05）；研究组并发症发生率为3.33%，低于对照组的23.33%（P＜0.05）；研究组治疗后IL-6、CRP水平低于对照组（P＜0.05）；研究组结石清除率为96.67%，高于对照组的66.67%（P＜0.05）。结论 输尿管软镜下钬激光碎石术治疗输尿管上段结石的效果确切，可缩短手术及康复时间，减轻患者炎症反应，降低术后并发症发生风险。
Abstract:
Objective To analyze the effect of flexible ureteroscopic holmium laser lithotripsy in the treatment of upper ureteral calculi. Methods A total of 50 patients with upper urinary tract calculi admitted to Suichuan County Hospital of Traditional Chinese Medicine from January to December 2023 were divided into control group and observation group by random number table method, with 25 patients in each group. The control group was treated with rigid ureteroscopy lithotripsy, and the observation group was treated with holmium laser lithotripsy under flexible ureteroscope. The therapeutic effect, operation and rehabilitation indexes (lithotripsy time, operation time, hospitalization time, double J tube indwelling time), incidence of complications, levels of inflammatory factors [interleukin-6 (IL-6), C-reactive protein (CRP)] and stone removal effect were compared between the two groups. Results The total effective rate of treatment in the study group (96.67%) was higher than that in the control group (73.33%) (P<0.05). Compared with the control group, the gravel time, operation time, hospitalization time and double J tube indwelling time in the study group were shorter (P<0.05). The incidence of complications in the study group (3.33%) was lower than that in the control group (23.33%) (P<0.05). Compared with control group, the levels of IL-6 and CRP in the study group were lower after treatment (P<0.05). The stone clearance rate of the study group (96.67%) was higher than that of the control group (66.67%) (P<0.05). Conclusion Flexible ureteroscopic holmium laser lithotripsy is effective in the treatment of upper ureteral calculi, which can shorten the operation and rehabilitation time, reduce the inflammatory response and reduce the risk of postoperative complications.
