摘要:
目的 了解国内外关节挛缩的研究热点和趋势,为相关研究与临床治疗提供参考。方法 以关节挛缩为主题进行检索2003年1月-2023年8月知网、万方、维普及PubMed和Web of Science的相关文献,运用NoteExpress 3.2软件查重、筛选、处理文献,使用Excel 2016统计发文量并制作表格,采用CiteSpace 6.2.R4软件绘制关键词、研究机构、来源期刊及作者知识图谱。结果 中文文献在2020-2021年分布呈现骤升,英文文献在2017-2018年骤升至第一个高峰,2018-2020年回降,2020-2021年骤升至最高点;中文纳入文献501篇,外文纳入文献265篇。关键词分析显示,研究主要集中在关节挛缩的治疗与康复;最高频的关键词中、英文均为关节挛缩;关节挛缩中文文献关键词共现聚类图谱共形成11个聚类,外文文献形成5个聚类。研究机构分析显示,中文文献发文最多的是北京积水潭医院(15篇),外文文献发文最多是Hiroshima International University(19篇);来源期刊分析显示,中文文献主要有《中国矫形外科杂志》(12篇),外文来源期刊主要有Journal of Orthopaedic Research(15篇);作者分析显示,中文文献作者发文最多为张全兵和周云(10篇),外文文献作者发文最多为Junya Ozawa(17篇)。关节挛缩后的康复以及胎儿的先天性关节挛缩是中文文献的研究前沿热点,而外文文献的研究重点从疾病本身转变为关节挛缩后患者的社交能力和回归社会的程度。结论 国内外研究热点都集中在关节挛缩的治疗和预防,国内研究注重关节挛缩的非手术的干预措施,国外研究侧重关节挛缩的治疗手段,阐明手术方式和技术要点,并通过动物实验解释药物治疗、运动疗法、影响细胞的基因表达对关节挛缩的影响机制。
Abstract:
Objective To understand the research hotspots and trends of joint contracture at home and abroad, and to provide reference for related research and clinical treatment. Methods The related literatures of CNKI, Wanfang, Weipu Pubmed and Web of Science from January 2003 to August 2023 were searched with the theme of joint contracture. NoteExpress 3.2 software was used to check, screen and process the literatures. Excel 2016 was used to count the number of publications and make tables. CiteSpace 6.2.R4 software was used to draw keywords, research institutions, source journals and author knowledge maps. Results The distribution of Chinese literature in 2020-2021 showed a sharp rise. English literature rose to the first peak in 2017-2018, fell back in 2018-2020, and rose to the highest point in 2020-2021. A total of 501 literatures were included in Chinese and 265 literatures were included in foreign languages. Keyword analysis showed that the research mainly focused on the treatment and rehabilitation of joint contracture. The most frequent keywords in Chinese and English were key contractures; the keyword co-occurrence clustering map of joint contracture in Chinese literature formed 11 clusters, and foreign literature formed 5 clusters. The analysis of research institutions showed that Beijing Jishuitan Hospital (15 literatures) published the most Chinese literature, and Hiroshima International University (19 literatures) published the most foreign literature. The analysis of source journals showed that the Chinese literature mainly included Orthopedic Journal of China (12 literatures), and the foreign source journals mainly included Journal of Orthopaedic Research (15 literatures). The author′s analysis showed that ZHANG Quanbing and ZHOU Yun (10 literatures) were the most published in Chinese literature, and Junya Ozawa (17 literatures) was the most published in foreign literature. The rehabilitation after joint contracture and the congenital joint contracture of the fetus were the research hotspots of Chinese literature, while the research focus of foreign literature had changed from the disease itself to the social ability and the degree of return to society of patients after joint contracture. Conclusion The research hotspots at home and abroad are focused on the treatment and prevention of joint contracture. Domestic research focuses on non-surgical intervention measures for joint contracture. Foreign research focuses on the treatment of joint contracture, clarifies the surgical methods and technical points, and explains the mechanism of drug therapy, exercise therapy, and gene expression affecting cells on joint contracture through animal experiments.
