目的 运用数据挖掘技术探讨艾灸治疗药物引起恶心呕吐的选穴规律。方法 检索中国知识资源总库(CNKI)、维普数据库(VIP)、万方数据库(Wanfang)、中国生物医学文献数据库(SinoMed)、Cochrane Library、PubMed、Embase建库至 2024 年 2 月 20 日收录的有关艾灸治疗药物引起恶心呕吐的临床研究文献,利用Excel 2021建立用穴处方数据库,采用SPSS Modeler 18.0、SPSS Statistics 26.0软件对穴位进行频数、部位、关联规则、聚类分析,探究艾灸治疗药物引起恶心呕吐选穴特点及规律。结果 共纳入120篇文献,包含35个穴位,总频次359次。高频穴位为足三里、中脘、神阙等,常用经脉为任脉、足阳明胃经,穴位大部分位于胸腹部和下肢。按照支持度排序,排名前4的组合为“足三里-中脘”“足三里-内关”“足三里-神阙-中脘”“足三里-内关-中脘”,并得到 3 个有效聚类群。结论 艾灸治疗药物引起恶心呕吐的选穴遵循调节脾胃、扶正祛邪、鼓舞元气的治疗原则,核心穴组为“足三里-神阙-中脘”。
Abstract:
Objective To explore the acupoint selection patterns of moxibustion for drug-induced nausea and vomiting based on data mining technology. Methods CNKI, VIP, Wanfang, SinoMed, Cochrane Library, PubMed and Embase were searched for clinical research literature on moxibustion treatment of drug-induced nausea and vomiting from the establishment of the database to February 20, 2024, and Excel 2021 was used to establish the acupoint prescription database. SPSS Modeler 18.0 and SPSS Statistics 26.0 software were used to analyze the frequency, location, association rules and cluster analysis of acupoints, so as to explore the characteristics and rules of acupoint selection for nausea and vomiting caused by moxibustion treatment drugs. Results A total of 120 papers were included, containing 35 acupoints with a total frequency of 359 times. The high-frequency acupoints were Zusanli, Zhongwan, and Shenque, etc. The commonly used meridians were the Ren meridian and the stomach meridian of foot-yangming, and most of the acupoints were located in the chest, abdomen, and lower limbs. The top four combinations in terms of support were "Zusanli-Zhongwan" "Zusanli-Neiguan" "Zusanli-Shenque-Zhongwan" and "Zusanli-Shenque-Zhongwan", and three effective clusters were obtained. Conclusion The acupoint selection for moxibustion in treating drug-induced nausea and vomiting follows the therapeutic principles of regulating spleen-stomach function, reinforcing healthy qi while eliminating pathogens, and revitalizing primordial qi. The core acupoint combination is "Zusanli-Shenque-Zhongwan."
