摘要:
目的 探究氨茶碱与甲泼尼龙琥珀酸钠联合治疗慢性阻塞性肺病急性加重期的临床效果及对血清CRP水平、PaO2的影响。方法 选取玉山县黄家驷医院2021年5月-2024年5月收治的慢性阻塞性肺病急性加重期患者60例作为研究对象,按照随机数字表法将其分为对照组和研究组,每组30例。对照组使用氨茶碱治疗,研究组在对照组基础上应用甲泼尼龙琥珀酸钠治疗。比较两组临床效果、肺功能[第1秒用力肺活量(FEV1)、用力肺活量(FCV)、第1秒用力肺活量占用力肺活量比值(FEV1/FCV)]、血清炎症因子水平[肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、降钙素原(PCT)、C反应蛋白(CRP)]、血气指标水平[pH、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)]。结果 研究组治疗总有效率高于对照组(P<0.05)。研究组肺功能指标均优于对照组(P<0.05)。研究组血清炎症因子水平均低于对照组(P<0.05)。研究组PaCO2水平低于对照组,PaO2水平高于对照组(P<0.05)。结论 氨茶碱与甲泼尼龙琥珀酸钠联合治疗慢性阻塞性肺病急性加重期患者的临床效果确切,能够改善患者肺功能,减轻患者炎症反应,改善临床效果及血气指标水平,值得临床应用。
Abstract:
Objective To explore the clinical effect of aminophylline combined with methylprednisolone sodium succinate in the treatment of acute exacerbation of chronic obstructive pulmonary disease and its effect on serum CRP level and PaO2. Methods A total of 60 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to Yushan County Huangjiasi Hospital from May 2021 to May 2024 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 30 patients in each group. The control group was treated with aminophylline, and the study group was treated with methylprednisolone sodium succinate on the basis of the control group. The clinical effect, lung function [forced vital capacity in the first second (FEV1), forced vital capacity (FCV), ratio of forced vital capacity in the first second to forced vital capacity (FEV1/FCV)], serum inflammatory factor levels [tumor necrosis factor-α (TNF-α), interleukin-8 (IL-8), procalcitonin (PCT), C-reactive protein (CRP)], blood gas index levels [pH, arterial partial pressure of carbon dioxide (PaCO2), arterial partial pressure of oxygen (PaO2)] were compared between the two groups. Results The total effective rate of treatment in the study group was higher than that in the control group (P<0.05). The lung function indexes of the study group were better than those of the control group (P<0.05). The levels of serum inflammatory factors in the study group were lower than those in the control group (P<0.05). The level of PaCO2 in the study group was lower than that in the control group, while the level of PaO2 was higher than that in the control group (P<0.05). Conclusion The combination of aminophylline and methylprednisolone sodium succinate has a definite clinical effect in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease, which can improve the lung function of patients, reduce the inflammatory response of patients, improve the clinical effect and blood gas index level, and is worthy of clinical application.
