目的 通过脑卒中高危人群筛查,分析脑卒中主要危险因素流行现状及一般人口学分布特征,为兴安盟地区脑卒中高危人群一级预防提供策略依据。方法 2023年在内蒙古兴安盟抽取社区(城市人口)和乡镇(农村人口)各一个项目筛查点,对监测点内居住半年以上的613名常住人口进行筛查,通过评估表评估、体格检查及实验室检测收集数据,比较主要危险因素在性别、年龄方面的差异。结果 男性高危占比(25.66%)高于女性(18.86%),随年龄增加,高危人群比例越高(P<0.05)。脑卒中8项危险因素中的前3项检出率从高到低依次为血脂异常(40.46%)、高血压(24.80%)、肥胖(23.98%);脑卒中高危人群相关危险因素前3项检出率从高到低依次为高血压(76.34%)、血脂异常(73.28%)、肥胖(59.54%)。男性高血压、肥胖、吸烟史检出率高于女性,差异有统计学意义(P<0.05);糖尿病、高血压的检出率随年龄增长而增加(P<0.05)。结论 兴安盟地区男性、高龄者的脑卒中危险因素检出率高,检出率前3位的危险因素为高血压、血脂异常、肥胖。对此,应加强全民健康教育,切实推动执行慢性病患者的“三高”管理,进而预防及减少兴安盟居民脑卒中的发生。
Abstract:
Objective To analyze the epidemic status and general demographic distribution characteristics of major risk factors of stroke through screening of high-risk population of stroke, and to provide strategic basis for primary prevention of high-risk population of stroke in Xing’an League. Methods In 2023, a project screening site was selected in each community ( urban population ) and township (rural population) in Xing’an League, Inner Mongolia, and 613 permanent residents living in the monitoring site for more than half a year were screened. Data were collected through assessment form evaluation, physical examination and laboratory testing to compare the differences in gender and age of the main risk factors. Results The proportion of high-risk males (25.66%) was higher than that of females (18.86%), and the proportion of high-risk groups increased with age (P<0.05). The top three risk factors of stroke were dyslipidemia (40.46%), hypertension (24.80%) and obesity (23.98%). The top three detection rates of related risk factors in high-risk population of stroke from high to low were hypertension (76.34%), dyslipidemia (73.28%) and obesity (59.54%). The detection rates of hypertension, obesity and smoking history in males were higher than those in females, and the differences were statistically significant (P<0.05). The detection rates of diabetes and hypertension increased with age (P<0.05). Conclusion The detection rate of stroke risk factors in male and elderly people in Xing’an League is high, and the top three risk factors are hypertension, dyslipidemia and obesity. It is necessary to strengthen national health education and effectively promote the implementation of "three high" management of patients with chronic diseases, which may effectively prevent and reduce the occurrence of stroke in Xing’an League residents.
