目的 分析依达拉奉右莰醇联合静脉溶栓治疗急性缺血性脑卒中(AIS)血管开通后的AIS患者神经功能改善及脑出血转化情况,探讨依达拉奉右莰醇对脑细胞的保护作用,及对静脉溶栓血管开通后再灌注损伤的影响,旨在寻求卒中治疗新方法。方法 选择上海市松江区泗泾医院2023年1月-2024年12月静脉溶栓血管开通的AIS患者100例,采用临床随机、开放标签的方法分为观察组50例和对照组50例。对照组在常规治疗基础上应用阿替普酶静脉溶栓,观察组阿替普酶静脉溶栓前即刻静脉滴注依达拉奉右莰醇注射液,比较两组患者治疗前、治疗后NIHSS评分、mRS评分,临床疗效,以及脑出血转化。结果 两组治疗后NIHSS评分、mRS评分均低于治疗前,观察组低于对照组(P<0.05);观察组总有效率为84.00%,高于对照组的74.00%,差异有统计学意义(P<0.05);观察组脑出血转化发生率(2.00%)低于对照组(6.00%),差异有统计学意义(P<0.05)。结论 依达拉奉右莰醇联合静脉溶栓较静脉溶栓单独治疗AIS能有助于改善AIS患者的神经功能,提高临床疗效,减少脑出血转化。
Abstract:
Objective To analyze the improvement of neurological function and cerebral hemorrhage transformation in patients with acute ischemic stroke (AIS) after recanalization of blood vessels treated with edaravone and dexborneol combined with intravenous thrombolysis, and to explore the protective effect of edaravone and dexborneol on brain cells and the effect on reperfusion injury after recanalization of blood vessels treated with intravenous thrombolysis, so as to seek a new method for stroke treatment. Methods From January 2023 to December 2024, 100 patients with AIS who underwent intravenous thrombolysis in Sijing Hospital of Songjiang District, Shanghai were selected and divided into observation group (50 patients) and control group (50 patients) by clinical randomized and open label method. The control group was treated with intravenous thrombolysis with alteplase on the basis of routine treatment. The observation group was treated with intravenous infusion of edaravone and dexborneol injection immediately before intravenous thrombolysis with alteplase. The NIHSS score, mRS score, clinical efficacy, and cerebral hemorrhagic transformation were compared between the two groups before and after treatment. Results After treatment, the NIHSS score and mRS score of the two groups were lower than those before treatment, and those of the observation group were lower than those of the control group (P<0.05). The total effective rate of the observation group was 84.00%, which was higher than 74.00% of the control group, and the difference was statistically significant (P<0.05). The incidence of hemorrhagic transformation in the observation group (2.00%) was lower than that in the control group (6.00%), and the difference was statistically significant (P<0.05). Conclusion Compared with intravenous thrombolysis alone, edaravone and dexborneol combined with intravenous thrombolysis can help improve the neurological function of AIS patients, improve clinical efficacy, and reduce cerebral hemorrhagic transformation.
