摘要:
目的 研究血浆纤维蛋白原和D-二聚体在急性缺血性脑卒中治疗中的应用价值。方法 选取2020年5月-2021年5月在我院诊治的90例急性缺血性脑卒中患者为研究对象，观察不同时间段进展性脑梗死和非进展性脑梗死、出院结局良好和不良患者的血浆纤维蛋白原（FIB）、D-二聚体（DD）水平和神经功能缺损评分。结果 进展性脑梗死发生率为31.11%，进展性脑梗死患者入院24 h、3 d、7 d FIB、DD水平均高于非进展性脑梗死患者，且住院7 d内FIB、DD水平均呈逐渐升高趋势（P＜0.05）；结局不良患者发生率为22.22%，结局不良患者入院24 h、3 d、7 d FIB、DD水平均高于结局良好患者（P＜0.05）；结局良好患者神经功能缺损（NIHSS）评分低于结局不良患者（P＜0.05）。结论 FIB、DD在入院和住院期间维持较高水平，可能预示急性缺血性脑卒中患者病情进展、不良结局，并且其水平与中枢神经系统功能受损有一定关系。
Abstract:
Objective To study the application value of plasma fibrinogen and D-dimer in the treatment of acute ischemic stroke.Methods A total of 90 patients with acute ischemic stroke diagnosed and treated in our hospital from May 2020 to May 2021 were selected as study subjects. Plasma fibrinogen (FIB), D-dimer (DD) levels and neurological deficit scores were observed in patients with progressive cerebral infarction and non-progressive cerebral infarction at different time periods, good and poor discharge outcomes.Results The incidence of progressive cerebral infarction was 31.11%, the levels of FIB and DD in patients with progressive cerebral infarction were higher than those in patients with non-progressive cerebral infarction at 24 h, 3 d and 7 d after admission, and the levels of FIB and DD in patients with progressive cerebral infarction were gradually increased within 7 d after admission (P<0.05). The incidence of patients with poor outcome was 22.22%, the levels of FIB and DD in patients with poor outcome were higher than those in patients with good outcome at 24 h, 3 d and 7 d after admission (P<0.05); the NIHSS score of patients with good outcome was lower than that of patients with poor outcome (P<0.05).Conclusion Plasma FIB and DD maintain a high level during admission and hospitalization, which may indicate the progression and adverse outcomes of patients with acute ischemic stroke, and their levels are related to the impairment of central nervous system function.
