摘要:
目的 探究安徽省宿州市中老年体检人群幽门螺杆菌(Hp)感染情况与新型炎症指标之间的相关性。方法 回顾性分析2016年1月4日-2023年12月10日在安徽医科大学附属宿州医院(宿州市立医院)体检中心体检、自愿进行碳呼气试验检测Hp感染情况且年龄≥45岁的成人,共计29 445例。收集其一般资料及血生化指标,比较不同性别、年龄组人群Hp感染率;再将患者分为Hp感染阳性组与阴性组,比较两组一般指标和新型炎症指标水平,将有差异的新型炎症指标纳入多因素Logistic回归分析,探索Hp感染与新型炎症指标的相关性。结果 29 445例研究对象中,总体Hp感染率为35.01%,其中男性Hp感染率为35.82%,高于女性的33.74%;中年人群(45~59岁)Hp感染率为35.89%,高于老年人群(≥60岁)的33.18%,差异均具有统计学意义(P<0.05)。在总体人群中,与Hp感染阴性组相比,Hp感染阳性组的体重指数、收缩压、舒张压、空腹血糖、甘油三酯、血红蛋白、白细胞、中性粒细胞计数、淋巴细胞计数、单核细胞计数均较高,而新型炎症指标中系统免疫性炎症指数(SII)、中性粒细胞与淋巴细胞计数比值(NLR)及系统性免疫炎症指数(SIRI)较高,血小板与淋巴细胞比值(PLR)及单核细胞与淋巴细胞比值(MLR)较低,差异均有统计学意义(P<0.05)。将新型炎症指标纳入多因素Logistic回归分析,在调整混杂因素后发现NLR与Hp感染呈正相关,PLR、MLR与Hp感染呈负相关。男性体检人群中,NLR与Hp感染呈正相关,PLR、MLR与Hp感染呈负相关;女性体检人群中,NLR与Hp感染呈正相关,PLR与Hp感染呈负相关。结论 在中老年人群中,Hp感染与新型炎症指标密切相关,其中NLR在中老年人群Hp感染筛查中更值得关注,可作为低成本筛查Hp感染风险的有效辅助工具。
Abstract:
Objective To explore the correlation between Helicobacter pylori (Hp) infection and new inflammatory markers in middle-aged and elderly people undergoing physical examination in Suzhou City, Anhui Province. Methods From January 4, 2016 to December 10, 2023, a total of 29 445 adults aged ≥45 years who underwent physical examination and voluntary carbon breath test to detect Hp infection in the physical examination center of Suzhou Hospital Affiliated to Anhui Medical University (Suzhou Municipal Hospital) were retrospectively analyzed. The general data and blood biochemical indexes were collected, and the Hp infection rates of different genders and age groups were compared. The patients were divided into Hp infection positive group and Hp infection negative group. The levels of general indexes and new inflammatory indexes were compared between the two groups. The different new inflammatory indexes were included in multivariate Logistic regression analysis to explore the correlation between Hp infection and new inflammatory indexes. Results Among the 29 445 subjects, the overall Hp infection rate was 35.01%, and the Hp infection rate of males was 35.82%, which was higher than that of females (33.74%). The Hp infection rate of middle-aged people (45-59 years old) was 35.89%, which was higher than 33.18% of the elderly (≥60 years old), and the difference was statistically significant (P<0.05). In the general population, compared with the Hp infection negative group, the body mass index, systolic blood pressure, diastolic blood pressure, fasting blood glucose, triglyceride, hemoglobin, white blood cell, neutrophil count, lymphocyte count and monocyte count of the Hp infection positive group were higher, while the systemic immune inflammation index (SII), neutrophil to lymphocyte count ratio (NLR) and systemic immune inflammation index (SIRI) were higher, platelet to lymphocyte ratio (PLR) and monocyte to lymphocyte ratio (MLR) were lower, and the differences were statistically significant (P<0.05). The new inflammatory markers were included in multivariate logistic regression analysis, after adjusting for confounding factors, it was found that NLR was positively correlated with Hp infection, and PLR and MLR were negatively correlated with Hp infection. In male physical examination population, NLR was positively correlated with Hp infection, while PLR and MLR were negatively correlated with Hp infection. In the female physical examination population, NLR was positively correlated with Hp infection, and PLR was negatively correlated with Hp infection.Conclusion In the middle-aged and elderly population, Hp infection is closely related to new inflammatory markers. Among them, NLR is more worthy of attention in the screening of Hp infection in the middle-aged and elderly population, and can be used as an effective auxiliary tool for low-cost screening of Hp infection risk.
