摘要:
目的 观察中医外治法结合微波腔内闭合术治疗下肢静脉曲张伴溃疡的临床效果。方法 选取2018年1月~2019年1月在我院采用微波腔内闭合术治疗的下肢静脉曲张伴溃疡患者90例，将其随机分为对照组和观察组，各45例。对照组手术前后采用常规治疗，观察组采用中医外治法（熏洗疗法、中药化腐清创术、湿敷疗法、掺药法）。比较两组治疗2个月后疗效、溃疡愈合时间、溃疡愈合面积、临床症状评分（疼痛、肿胀、溃疡、渗液、行走恢复、皮肤）、并发症发生率。结果 观察组治疗总有效率为88.89%，高于对照组的71.11%，差异有统计学意义（P＜0.05）；观察组溃疡愈合时间短于对照组、溃疡愈合面积小于对照组，差异均有统计学意义（P＜0.05）；观察组临床症状评分低于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率低于对照组，但差异无统计学意义（P＞0.05）。结论 中医外治法结合微波腔内闭合术治疗下肢静脉曲张伴溃疡，疗效确切，溃疡愈合率高，缩短了愈合时间，降低临床并发症发生率，是一种安全有效的治疗方法。
Abstract:
Abstract：Objective To observe the clinical effect of external treatment of traditional Chinese medicine combined with microwave intracavitary closure in the treatment of lower limb varicose veins with ulcer. Methods 90 patients with lower limb varicose veins treated with microwave endovascular closure in our hospital from January 2018 to January 2019 were enrolled. They were randomly divided into a control group and an observation group, 45 cases each. The control group received routine treatment before and after surgery, and the observation group used traditional Chinese medicine treatment (smoked and washed therapy, Chinese medicine rot debridement, wet compress therapy, and drug-filling method). The efficacy, ulcer healing time, ulcer healing area, clinical symptom score (pain, swelling, ulcer, exudate, walking recovery, skin) and complication rate were compared between the two groups after 2 months of treatment. Results The total effective rate of the observation group was 88.89%, which was higher than that of the control group 71.11%,the difference was statistically significant (P<0.05). The ulcer healing time of the observation group was shorter than that of the control group and the ulcer healing area was smaller than that of the control group,the difference was statistically significant (P<0.05). The clinical symptom score of the observation group was lower than that of the control group,the difference was statistically significant (P<0.05). The incidence of complications in the observation group was lower than that of the control group, but the difference was not statistically significant (P>0.05).Conclusion TCM external treatment combined with microwave endovascular closure for the treatment of lower limb varicose veins with ulcers is effective, the ulcer healing rate is high, the healing time is shortened, and the incidence of clinical complications is reduced. It is a safe and effective treatment.
