摘要:
目的 推广按病种付费模式是现今医院管理中较为新型的一类医保支付模式，本文主要就清远市按病种付费模式的应用及价值进行分析。方法 对院内接诊病患按照ICD-10进行分类，选取临床诊疗中较为常见的实施外科手术治疗的疾病类型（白内障、肝胆管结石、急性阑尾炎等），纳入2012年4月~2013年11月收治的上述疾病患者95例设为对照组，纳入2015年7月~2018年8月收治的上述疾病患者95例设为研究组，对两组患者基线资料、住院治疗情况及费用、药品使用情况等进行综合统计并对比。结果 观察组患者不同病种的住院治疗时间均短于对照组，住院治疗费用及药品费用均低于对照组，差异具有统计学意义（P＜0.05）。两组患者在治疗费、材料费、手术费方面的占比比较，差异无统计学意义（P＞0.05）；观察组患者药品费占比低于对照组，差异具有统计学意义（P＜0.05）。结论 按病种付费模式在降低病患诊疗费用、药品费占比，缩短治疗时间方面均具有较为显著的效果，能够显著提升医院管理的质量。
Abstract:
Abstract：Objective To promote the mode-based payment model is a relatively new type of medical insurance payment model in hospital management. This paper mainly evaluates the application and value of the disease-based payment model in Qingyuan City. Methods The patients admitted to the hospital were classified according to ICD-10, and the types of diseases (cataract, hepatolithiasis, acute appendicitis, etc.) that were more commonly used in clinical treatment were selected. April 2012~November 2013,95 patients with the above-mentioned diseases were enrolled in the control group. 95 patients with the above-mentioned diseases admitted from July 2015 to August 2018 were enrolled in the study group. Baseline data, hospitalization, expenses, and drug use were compared between the two groups. Wait for comprehensive statistics and comparison.Results The hospitalization time of different disease groups in the observation group was shorter than that of the control group. The hospitalization cost and drug cost were lower than the control group,the difference was statistically significant (P<0.05). There was no significant difference in the proportion of treatment, materials and surgery between the two groups (P>0.05). The proportion of drug expenses in the observation group was lower than that in the control group,the difference was statistically significant (P<0.05). Conclusion The disease-based payment model has a significant effect in reducing the cost of medical treatment, the proportion of medicines, and shortening the treatment time, which can significantly improve the quality of hospital management.
