摘要:
目的 探讨老年及中青年人群血脂水平与冠心病的相关性。方法 选取2016年9月~2019年9月深圳市龙岗中心医院心内科接受冠脉造影检查的患者654例,根据冠脉造影结果分为冠心病组(420例)和非冠心病组(234例),另根据年龄将各组患者分为年龄≥60岁和年龄<60岁,比较两组不同年龄段患者血脂水平[总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)],并采用多因素Logistic回归分析不同年龄段CHD的影响因素。结果 两组<60岁年龄段TG、HDL-C水平比较,差异无统计学意义(P>0.05);冠心病组<60岁年龄段TC、LDL-C水平高于非冠心病组,差异有统计学意义(P<0.05);两组≥60岁年龄段TG、TC、HDL-C、LDL-C水平比较,差异均无统计学意义(P>0.05)。<60岁年龄段CHD患者TG、TC、HDL-C、LDL-C水平异常占比高于≥60岁年龄段CHD患者,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,CHD患者中血脂水平以LDL-C的升高是CHD发生的危险因素(P<0.05);在<60岁年龄段患者中LDL-C的升高是CHD发生的危险因素(P<0.05);在≥60岁年龄段患者中TG、TC、HDL-C、LDL-C水平对CHD发生无影响(P>0.05)。结论 血脂四项中LDL-C与CHD发生有关,LDL-C的升高又与中青年CHD发生相关,但其在老年人群中的意义有限。
Abstract:
Objective To explore the correlation between serum lipid levels and coronary heart disease in elderly, middle-aged and young people. Methods From September 2016 to September 2019, 654 patients who underwent coronary angiography in the Department of Cardiology of Shenzhen Longgang Central Hospital were selected and divided into coronary heart disease group (420 cases) and non- coronary heart disease group (234 cases) according to the results of coronary angiography. In addition, the patients in each group were divided into ≥60 years of age and <60 years of age according to age,comparison of blood lipid levels [total cholesterol (TC), triacylglycerol (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C)] of patients of different age groups in the two groups, using multiple factors Logistic regression analysis of influencing factors of CHD in different age groups.Results There was no significant difference in the levels of TG and HDL-C in the age group of <60 years (P>0.05); The levels of TC and LDL-C in the coronary heart disease group <60 years of age were higher than those in the non-coronary heart disease group,the differences were statistically significant (P<0.05);There was no significant difference in the levels of TG, TC, HDL-C, and LDL-C between the two groups ≥60 years of age (P>0.05).The proportion of abnormal levels of TG, TC, HDL-C, and LDL-C in patients with CHD <60 years of age was higher than that in patients with CHD≥60 years of age,the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that the increase in LDL-C was the risk factor for CHD in patients with CHD(P<0.05); The increase of LDL-C is a risk factor for CHD in patients <60 years of age (P<0.05);The levels of TG, TC, HDL-C, and LDL-C had no effect on the occurrence of CHD in patients aged ≥60 years(P>0.05).Conclusion LDL-C is related to the occurrence of CHD in the four blood lipids, and the increase of LDL-C is related to the occurrence of CHD in young and middle-aged people, but its significance in the elderly is limited.
