摘要:
目的 分析X线检查在颈椎失稳中的诊断价值。方法 选取2019年1月~10月因颈部不适来我院门诊就诊的患者108例,选取颈椎病患者44例为颈椎病组,选取颈肩疼痛3个月以上患者64例为症状组,另选取同期我院体检健康者27名为对照组。使用同一台X线摄影机拍摄其过屈位、中立位、过伸位平片并计算最大活动度,比较各组不同体位颈椎曲度指数及最大活动度,记录各组双边征及骨赘样增生情况。结果 三组过屈位颈椎曲度指数比较,差异无统计学意义(P>0.05);症状组与颈椎病组中立位、过伸位颈椎曲度指数及最大活动度与正常组比较,差异有统计学意义(P<0.05),但与颈椎病组比较,差异无统计学意义(P>0.05);颈椎病组出现双边征15例,症状组出现双边征13例,正常组出现双边征0例;颈椎病组出现骨赘样增生18例,症状组出现骨赘样增生15例,正常组出现骨赘样增生4例。结论 动态X线平片可以很好的反映出因颈椎失稳引发的曲度变化最大活动度、双突双边征及骨赘样增生等问题,临床对颈椎曲度异常要引起足够重视。
Abstract:
Objective To analyze the diagnostic value of X - ray examination in cervical spine instability.Methods From January to October 2019, 108 patients who came to our clinic for cervical discomfort were selected, 44 patients with cervical spondylosis were selected as the cervical spondylopathy group, and 64 patients with neck and shoulder pain for more than 3 months were selected as the symptom group. During the same period, 27 healthy people in our hospital were the control group.The same X-ray camera was usedto take plain films of flexion position, neutral position, hyperextension position and calculate the maximum range of motion.The index in cervical spine curvature were compared between different groups, and bilateral signs and osteophyte hyperplasia were recorded.Results There was no significant difference in the cervical flexion index between the three groups in flexion position(P>0.05). The difference of cervical curvature index and maximum range of motion in neutral position and hyperextension position between the symptom group and the cervical spondylosis group was statistically significant(P<0.05), but there was no statistically significant difference between the symptom group and the cervical spondylosis group(P>0.05); 15 cases of bilateral signs appeared in the cervical spondylosis group, and the symptom group were 13 cases of bilateral signs , 0 case in the normal group; 18 cases of osteophyte hyperplasia in the cervical spondylosis group,15 cases showed osteophyte hyperplasia in the symptom group and 4 cases showed osteophyte hyperplasia in the normal group.Conclusion Dynamic X-ray films can well reflect the problems of curvature changes caused by cervical instability, bilateral signs and osteophyte hyperplasia. It is necessary to pay enough attention to abnormal cervical curvature.
