摘要:
目的 观察不同用药方案治疗幽门螺杆菌（Hp）感染胃溃疡患者的临床效果。方法 选择2018年4月~2019年4月我院收治的90例Hp感染胃溃疡患者作为研究对象，按照治疗方式分为对照组和观察组，各45例。对照组采用阿莫西林、奥美拉唑联合克林霉素三联疗法治疗，观察组采用阿莫西林、奥美拉唑联合克林霉素序贯疗法治疗，比较两组Hp根除率、临床疗效、治疗前后炎症因子水平[C-反应蛋白（CRP ）、白细胞介素-1（IL-1）、白细胞介素-6（IL-6）]及胃泌素族指标[胃泌素（GAS）、胆囊收缩素（CCK）及胃动素（MTL）]。结果 治疗后，两组炎症因子（CRP、IL-1、IL-6）及胃泌素族指标（GAS、CCK、MTL）水平均较治疗前降低，且观察组各项指标改善程度优于对照组，差异有统计学意义（P＜0.05）；观察组Hp根除率为93.33%，治疗总有效率为95.56%，均高于对照组的75.56%、77.78%，差异有统计学意义（P＜0.05）。结论 序贯疗法治疗Hp感染胃溃疡临床疗效确切，可有效改善患者血清炎症因子及胃液中的胃泌素族指标水平，对Hp的根除及胃溃疡症状的改善有积极作用。
Abstract:
Objective To observe the clinical effects of different medication regimens in the treatment of patients with gastric ulcer infection by Helicobacter pylori (Hp).Methods 90 patients with Hp infection gastric ulcer admitted to our hospital from April 2018 to April 2019 were selected as the research objects. According to the treatment method, they were divided into control group and observation group, with 45 cases in each group. The control group was treated with amoxicillin, omeprazole combined with clindamycin triple therapy, and the observation group was treated with amoxicillin, omeprazole combined with clindamycin sequential therapy. The Hp eradication rate and clinical efficacy of the two groups were compared,levels of inflammatory factors before and after treatment [C-reactive protein (CRP), interleukin-1 (IL-1), interleukin-6 (IL-6)] and gastrin family indicators [gastrin (GAS) , Cholecystokinin (CCK) and motilin (MTL)].Results After treatment, the levels of inflammatory factors (CRP, IL-1, IL-6) and gastrin group indicators (GAS, CCK, MTL) in the two groups were lower than before treatment, and the improvement of various indicators in the observation group was better than that in the control group,the difference was statistically significant (P<0.05); the Hp eradication rate in the observation group was 93.33%, and the total effective rate of treatment was 95.56%, both higher than 75.56% and 77.78% in the control group,the difference was statistically significant (P<0.05).Conclusion Sequential therapy had a definite clinical effect in the treatment of Hp infection gastric ulcer. It could effectively improve the serum inflammatory factors and gastrin index levels in gastric juice. It had a positive effect on the eradication of Hp and the improvement of gastric ulcer symptoms.
