摘要:
目的 分析后侧入路内固定治疗累及胫骨平台后柱骨折患者的临床疗效。方法 选取2018年6月~2019年6月在我院诊治的100例累及胫骨平台后柱骨折患者，采用随机数字表法分为对照组和观察组，各50例。对照组采用前侧入路内固定治疗，观察组采用后侧入路内固定治疗，比较两组临床治疗优良率、手术指标（手术时间、住院时间、术中出血量、愈合时间）、膝关节活动度（屈膝度、伸膝度）、膝关节功能评分以及并发症（切口感染、内固定松动、血管神经损伤）发生情况。结果 观察组临床治疗总有效率为82.00%，高于对照组的70.00%（P＜0.05）；观察组手术时间、住院时间、术中出血量、愈合时间均大于对照组（P＜0.05）；治疗后观察组膝关节活动度均小于对照组，关节评分高于对照组（P＜0.05）；观察组并发症发生率低于对照组（P＜0.05）。结论 后侧入路内固定治疗累及胫骨平台后柱骨折患者疗效确切，骨折愈合优良率高，膝关节活动度大，膝关节功能评分高，且术后并发症少，具有显著的临床应用效果。
Abstract:
Objective To analyze the clinical efficacy of posterior approach internal fixation for the treatment of fractures of the posterior column of the tibial plateau.Methods A total of 100 patients with fractures of the posterior column of the tibial plateau who were diagnosed and treated in our hospital from June 2018 to June 2019 were selected and divided into a control group and an observation group using a random number table method, with 50 cases in each group. The control group was treated with anterior approach internal fixation, and the observation group was treated with posterior approach internal fixation. The clinical treatment excellent and good rate, surgical indicators (operation time, hospitalization time, intraoperative blood loss, healing time), knee joints were compared between the two groups mobility (knee flexion, knee extension), knee joint function score, and complications (incision infection, internal fixation loosening, vascular nerve injury) occurrence.Results The clinical excellent and good rate of the observation group was 82.00%, which was higher than 70.00% of the control group(P<0.05);The operation time, hospitalization time, intraoperative blood loss, and healing time of the observation group were greater than those of the control group(P<0.05);The range of motion of the knee joint in the observation group was lower than that in the control group, and the joint score was higher than that in the control group(P<0.05); the incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Posterior approach internal fixation was effective in treating patients with tibial plateau posterior column fractures, with high fracture healing rate, high knee mobility, high knee function score, and few postoperative complications. It was worthy of clinical application.
