摘要:
目的 研究关节镜结合玻璃酸钠关节腔注射治疗膝关节骨性关节炎临床疗效。方法 选取2017年10月~2020年10月在我院诊治的80例膝关节骨性关节炎患者为研究对象，采用随机数字表法分为对照组和观察组，各40例。对照组采用关节镜下清理术治疗，观察组在对照组基础上联合玻璃酸钠关节腔注射治疗，比较两组治疗总有效率、炎症指标、Ly-sholm评分、疼痛评分、临床症状评分以及并发症发生率。结果 观察组治疗总有效率为95.00%，高于对照组的82.50%，差异有统计学意义（P＜0.05）；治疗后两组IL-1β、IL-6、TNF-α水平均较治疗前降低，且观察组低于对照组，差异有统计学意义（P＜0.05）；治疗后Ly-sholm评分高于治疗前，疼痛评分、临床症状评分低于治疗前，且观察组Ly-sholm评分高于对照组，疼痛评分、临床症状评分低于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率为7.50%，高于对照组的17.50%，差异有统计学意义（P＜0.05）。结论 关节镜结合玻璃酸钠关节腔注射治疗膝关节骨性关节炎效果确切，可提高治疗有效率，降低炎症指标水平，促进炎症吸收，降低疼痛和临床症状评分，提高Ly-sholm评分，且可降低并发症发生率。
Abstract:
Objective To study the clinical efficacy of arthroscopy combined with sodium hyaluronate injection in the joint cavity for the treatment of knee osteoarthritis.Methods A total of 80 patients with knee osteoarthritis who were diagnosed and treated in our hospital from October 2017 to October 2020 were selected as the research objects. They were divided into the control group and the observation group by random number table method, with 40 cases in each group.The control group was treated with arthroscopic debridement, and the observation group was treated with sodium hyaluronate injection in the joint cavity on the basis of the control group.The total effective rate of treatment, inflammation index, Ly-sholm score, pain score, clinical symptom score and complication rate were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.00%, which was higher than 82.50% in the control group,the difference was statistically significant (P＜0.05);After treatment, the levels of IL-1β, IL-6 and TNF-α in the two groups were lower than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P＜0.05);After treatment, Ly-sholm score was higher than before treatment, pain score and clinical symptom score were lower than before treatment, and Ly-sholm score of observation group was higher than control group, pain score and clinical symptom score were lower than control group, the difference was statistically significant (P＜0.05);The incidence of complications in the observation group was 7.50%, which was higher than 17.50% in the control group,the difference was statistically significant (P＜0.05).Conclusion Arthroscopy combined with sodium hyaluronate injection in the joint cavity is effective in treating knee osteoarthritis.It can improve the effective rate of treatment, reduce the level of inflammation indicators, promote inflammation absorption, reduce pain and clinical symptom scores, improve Ly-sholm scores, and reduce the incidence of complications.
