摘要:
目的 观察单髁置换术治疗老年膝骨关节炎患者的效果及对膝关节功能的影响。方法 选取2015年11月~2020年11月我院治疗的80例老年膝骨关节炎患者为研究对象，采用随机数字表法分为观察组和对照组，各40例。对照组采用全膝关节置换术，观察组采用单髁置换术，比较两组手术相关指标、疼痛评分、膝关节功能评分、膝关节活动度、WOMAC骨关节炎指数及并发症发生情况。结果 观察组手术时间、术中出血量、住院时间均小于对照组，差异有统计学意义（P＜0.05）；观察组术后VAS评分低于对照组[（2.45±0.41）分 vs（4.98±0.31）分]，差异有统计学意义（P＜0.05）；观察组膝关节功能评分、膝关节活动度大于对照组，WOMAC骨关节炎指数低于对照组，差异有统计学意义（P＜0.05）；随访6个月，观察组并发症发生率为5.00%，与对照组的7.50%比较，差异无统计学意义（P＞0.05）。结论 单髁置换术治疗老年膝骨关节炎效果理想，手术时间短，术中出血量少，对患者创伤小，利于术后恢复，可缩短住院时间，同时可提高患者关节活动度，改善膝关节功能，术后并发症少。
Abstract:
Objective To observe the effect of unicondylar replacement in the treatment of elderly patients with knee osteoarthritis and its influence on knee joint function.Methods A total of 80 elderly patients with knee osteoarthritis treated in our hospital from November 2015 to November 2020 were selected as the research objects. They were divided into observation group and control group by random number table method, with 40 cases in each group.The control group underwent total knee arthroplasty, and the observation group underwent unicondyle replacement. The two groups were compared with surgical-related indicators, pain scores, knee function scores, knee range of motion, WOMAC osteoarthritis index, and complications.Results The operation time, intraoperative blood loss, and hospitalization time of the observation group were less than those of the control group,the difference was statistically significant (P<0.05);The postoperative VAS score of the observation group was lower than that of the control group[(2.45±0.41)scores vs (4.98±0.31)scores],the difference was statistically significant (P＜0.05);The observation group’s knee joint function score and knee joint range of motion were greater than those of the control group, and the WOMAC osteoarthritis index was lower than that of the control group, the difference was statistically significant (P<0.05);After 6 months of follow-up, the complication rate in the observation group was 5.00%, which was not statistically significant compared with 7.50% in the control group (P>0.05).Conclusion Unicondylar replacement surgery has an ideal effect in the treatment of elderly knee osteoarthritis. The operation time is short, the intraoperative blood loss is small, the trauma is small, and it is beneficial to postoperative recovery and shortens the hospital stay.At the same time, it can increase the patient’s joint range of motion, improve knee joint function, and reduce postoperative complications.
