摘要:
目的 观察Solitaire支架动脉取栓术治疗急性心源性脑栓塞临床效果。方法 选取2019年11月~2020年11月在我院治疗的66例急性心源性脑栓塞患者作为研究对象，采用随机数字表法分为对照组和观察组，各33例。对照组采用静脉溶栓治疗，观察组采用Solitaire支架动脉取栓术治疗。比较两组血管再通率、NIHSS评分、GCS评分、mRS评分、并发症发生率、死亡率及生命质量评分。结果 观察组血管再通率为87.87%，高于对照组的75.67%（P＜0.05）；两组NIHSS评分均低于术前，且观察组低于对照组（P＜0.05）；两组GCS评分均高于术前，且观察组高于对照组（P＜0.05）；观察组mRS评分低于对照组（P＜0.05）；两组并发症发生率、死亡率比较，基本一致（P＞0.05）；观察组生命质量各维度评分均高于对照组（P＜0.05）。结论 Solitaire支架动脉取栓术治疗急性心源性脑栓塞效果确切，可提高血管再通率，降低NIHSS评分，提高GCS评分，降低并发症发生率和mRS评分，促进患者良好预后形成，提高其生命质量。
Abstract:
Objective To observe the clinical effect of Solitaire stent arterial embolization in the treatment of acute cardiogenic cerebral embolism.Methods A total of 66 patients with acute cardiogenic cerebral embolism who were treated in our hospital from November 2019 to November 2020 were selected as the research objects. They were divided into the control group and the observation group by random number table method, with 33 cases in each group.The control group was treated with intravenous thrombolysis, and the observation group was treated with Solitaire stent arterial thrombectomy. The vascular recanalization rate, NIHSS score, GCS score, mRS score, complication rate, mortality rate and quality of life score were compared between the two groups.Results The recanalization rate of the observation group was 87.87%, which was higher than that of the control group, 75.67%(P＜0.05);The NIHSS scores of the two groups were lower than those before the operation, and the observation group was lower than the control group(P＜0.05);The GCS scores of the two groups were higher than those before the operation, and the observation group was higher than the control group(P＜0.05);The mRS score of the observation group was lower than that of the control group(P＜0.05);There was no statistically significant difference in the incidence of complications and mortality between the two groups (P>0.05);The scores of all dimensions of the quality of life of the observation group were higher than those of the control group(P<0.05).Conclusion Solitaire stent arterial thrombectomy is effective in the treatment of acute cardiogenic cerebral embolism, which can increase the recanalization rate, reduce the NIHSS score, increase the GCS score, reduce the incidence of complications and mRS score, promote the development of a good prognosis and improve the life of the patient quality.
