摘要:
目的 探讨慢性乙肝患者血清高尔基体蛋白73(GP73)与肝脏炎症及纤维化的相关性。方法 选择我院2017年1月~2019年6月收治121例肝活检确诊的慢性乙肝患者为观察对象,分析患者丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、白蛋白(ALB)、总胆红素(TBIL)、肝脏弹性检测(LSM)、FIB-4、APRI等肝组织病理炎症及纤维化指标与GP73的相关性,通过单因素和多因素分析法对血清GP73与肝脏炎症及纤维化的相关性进行统计分析。结果 血清GP73水平随肝脏炎症和肝纤维化程度加重而升高,与肝组织炎症及纤维化呈正相关(r=0.398、0.632、P<0.001);对显著肝脏炎症(≥G2)及纤维化(≥S2)分别进行单因素及多因素分析,结果显示,显著肝脏炎症及肝纤维化患者的血清GP73水平高于无或轻度肝脏炎症(G0-1)及纤维化患者(S0-1)(P<0.05);多变量Logistic回归分析结果显示,GP73可以作为显著肝脏炎症及纤维化的独立风险预测因素(P<0.05)。结论 血清GP73与慢性乙肝患者肝组织炎症和纤维化具有相关性,有可能作为预测肝组织炎症分级和纤维化分期的潜在标志物。
Abstract:
Objective To investigate the relationship between serum Golgi protein 73 (GP73) and liver inflammation and fibrosis in patients with chronic hepatitis B.Methods Selecting 121 patients with chronic hepatitis B diagnosed by liver biopsy in our hospital from January 2017 to June 2019 as the observation objects.Analysis of patients with alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin (ALB), total bilirubin (TBIL), liver elasticity test (LSM), FIB-4, APRI, etc. The correlation between liver tissue pathological inflammation and fibrosis indicators and GP73,the correlation between serum GP73 and liver inflammation and fibrosis was statistically analyzed by single factor and multivariate analysis. Results Serum GP73 level increased with the degree of liver inflammation and fibrosis, and was positively correlated with liver tissue inflammation and fibrosis (r=0.398, 0.632, P<0.001);Single-factor and multi-factor analysis were performed on significant liver inflammation (≥G2) and fibrosis (≥S2), respectively,the results showed that the serum GP73 level in the significant liver inflammation and liver fibrosis patients was significantly higher than that in the no or mild liver inflammation (G0-1) and fibrosis patients (S0-1) (P<0.05);Multivariate Logistic regression analysis showed that GP73 can be used as an independent risk predictor of significant liver inflammation and fibrosis (P<0.05).Conclusion Serum GP73 is correlated with liver inflammation and fibrosis in patients with chronic hepatitis B, and may be used as a potential marker for predicting liver inflammation and fibrosis stages.
