摘要:
目的 探讨射血分数保留性心力衰竭患者脑钠肽水平的临床意义及与左心房的相关性分析。方法 选取2018年1月-2019年12月我院收治的50例射血分数保留性心力衰竭患者作为研究组，其中心功能Ⅰ~Ⅱ级组18例、Ⅲ~Ⅳ级组32例，另选取同时期在我院体检的50名健康者作为对照组。比较研究组中Ⅰ~Ⅱ级、Ⅲ~Ⅳ级组及对照组cTn及BNP水平、心功能指标，并分析研究组患者BNP水平与心功能指标的关系。结果 Ⅲ~Ⅳ级组cTn及BNP水平高于Ⅰ~Ⅱ级组及对照组，且Ⅰ~Ⅱ级组高于对照组，差异有统计学意义（P＜0.05）；Ⅲ~Ⅳ级组左房前后内径及舒张末期容积、收缩末期容积均高于Ⅰ~Ⅱ级组及对照组，而左心室射血分数低于Ⅰ~Ⅱ级组及对照组，差异有统计学意义（P＜0.05）。Pearson相关性分析显示，研究组患者BNP与左房前后内径及舒张末期容积、收缩末期容积均呈正相关（r=0.655、0.628、0.498，P＜0.05），与左心室射血分数呈负相关（r=-0.617，P＜0.05）。结论 对射血分数保留性心力衰竭患者进行脑钠肽水平检测，能够对患者病情严重程度进行有效评估，且能对患者心功能状态进行有效反映，从而为临床诊断及治疗提供可靠的参考依据。
Abstract:
Objective To investigate the clinical significance of brain natriuretic peptide level in patients with heart failure with preserved ejection fraction and its correlation with left atrium.Methods A total of 50 patients with heart failure with preserved ejection fraction admitted to our hospital from January 2018 to December 2019 were selected as the study group, including 18 cases of cardiac function Ⅰ-Ⅱ and 32 cases of cardiac function Ⅲ-Ⅳ. Another 50 healthy people who underwent physical examination in our hospital at the same time were selected as the control group. The levels of cTn and BNP and cardiac function indexes in the study group and the control group were compared, and the relationship between BNP level and cardiac function indexes in the study group was analyzed.Results The levels of cTn and BNP in the Ⅲ-Ⅳ group were higher than those in the Ⅰ-Ⅱ group and the control group, and those in the Ⅰ-Ⅱ group were higher than those in the control group, and the difference was statistically significant (P<0.05). The left atrial diameter, end-diastolic volume and end-systolic volume of the Ⅲ-Ⅳ group were higher than those of the Ⅰ-Ⅱ group and the control group, while the left ventricular ejection fraction was lower than that of the Ⅰ-Ⅱ group and the control group, and the difference was statistically significant (P<0.05). Pearson correlation analysis showed that BNP in the study group was positively correlated with left atrial diameter, end-diastolic volume and end-systolic volume (r=0.655, 0.628, 0.498, P<0.05 ), and negatively correlated with left ventricular ejection fraction (r=-0.617, P<0.05).Conclusion The detection of brain natriuretic peptide level in patients with heart failure with ejection fraction retention can effectively evaluate the severity of the disease and reflect the cardiac function of patients, thus providing a reliable reference for clinical diagnosis and treatment.
