摘要:
目的 探讨炎症细胞因子超敏C反应蛋白(hs-CRP)、B型脑纳肽(BNP)、胱抑素C(Cys C)、同型半胱氨酸(HCY)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平变化与慢性心力衰竭(CHF)患者心室重构(VR)的关系。方法 选取2017年1月-2019年8月我院收治的CHF住院患者120例为研究组,按照中国心力衰竭诊断和治疗指南2018版心功能分级标准分为心功能Ⅱ级组40例、心功能Ⅲ级组40例、心功能Ⅳ级组40例;另选取同期非CHF患者40例为对照组。检测两组外周血hs-CRP、BNP、CysC、HCY、Lp-PLA2水平,同日采用超声心动图检测左心室舒张末内径(LVEDD)、左心室质量指数(LVMI)、左心室射血分数(LVEF)指标,并行相关性分析。结果 研究组hs-CRP、BNP、CysC、HCY、Lp-PLA2水平均高于对照组,差异有统计学意义(P<0.05);随着心功能分级的递增,hs-CRP、BNP、CysC、HCY、Lp-PLA2水平也随之升高(P<0.05);研究组LVEDD、LVMI均高于对照组,LVEF均低于对照组,差异有统计学意义(P<0.05);LVEDD、LVMI水平也随着心功能分级的递增成逐级递增趋势,LVEF水平成逐级递减趋势(P<0.05);hs-CRP、BNP、CysC、HCY、Lp-PLA2水平与LVEDD(r=0.405、0.614、0.543、0.498、0.613,P<0.05)、LVMI(r=0.457、0.565、0.572、0.506、0.566,P<0.05)均呈正相关,与LVEF呈负相关(r=-0.487、-0.608、-0.556、-0.516、-0.638,P<0.05)。结论 CHF患者hs-CRP、BNP、CysC、HCY、Lp-PLA2与CHF患者VR有关,其水平的升高反映CHF患者心功能的严重程度;说明炎症细胞因子在CHF患者VR中发挥重要作用,在CHF患者心脏康复治疗过程中,应注意纠正患者血清hs-CRP、BNP、CysC、HCY、Lp-PLA2水平,阻断VR。
Abstract:
Objective To investigate the relationship between the levels of inflammatory cytokines high-sensitivity C-reactive protein (hs-CRP), type B brain natriuretic peptide (BNP), cystatin C (CysC), homocysteine (HCY), lipoprotein-associated phospholipase A2 (Lp-PLA2) and ventricular remodeling (VR) in patients with chronic heart failure (CHF).Methods A total of 120 CHF inpatients admitted to our hospital from January 2017 to August 2019 were selected as the study group. According to the 2018 edition of the guidelines for the diagnosis and treatment of heart failure, they were divided into cardiac function Ⅱ group (n=40), cardiac function Ⅲ group (n=40) and cardiac function Ⅳ group (n=40). Another 40 non CHF patients were selected as control group. The levels of hs-CRP, BNP, CysC, HCY and Lp-PLA2 in peripheral blood of the two groups were detected. The left ventricular end-diastolic diameter (LVEDD), left ventricular mass index (LVMI) and left ventricular ejection fraction (LVEF) were detected by echocardiography on the same day, and the correlation analysis was performed.Results The levels of hs-CRP, BNP, CysC, HCY and Lp-PLA2 in the study group were higher than those in the control group, and the difference was statistically significant (P<0.05). With the increase of cardiac function classification, the levels of hs-CRP, BNP, CysC, HCY and Lp-PLA2 increased (P<0.05). LVEDD and LVMI in the study group were higher than those in the control group, and LVEF was lower than that in the control group, the difference was statistically significant (P<0.05). The levels of LVEDD and LVMI increased gradually with the increase of cardiac function classification, and the level of LVEF decreased gradually (P<0.05). hs-CRP, BNP, CysC, HCY, Lp-PLA2 levels were positively correlated with LVEDD (r=0.405, 0.614, 0.543, 0.498, 0.613, P<0.05), LVMI (r=0.457, 0.565, 0.572, 0.506, 0.566, P<0.05), and negatively correlated with LVEF (r=-0.487, -0.608, -0.556, -0.516, -0.638, P<0.05).Conclusion hs-CRP, BNP, CysC, HCY and Lp-PLA2 in CHF patients are related to VR in CHF patients, and the increase in their levels reflects the severity of cardiac function in CHF patients. It shows that inflammatory cytokines play an important role in VR of CHF patients. In the process of cardiac rehabilitation treatment of CHF patients, attention should be paid to correct serum hs-CRP, BNP, CysC, HCY, Lp-PLA2 levels and block VR.
