摘要:
目的 探讨腰椎椎旁肌肉脂肪化分级程度与腰痛评分的相关性以及腰背部肌肉锻炼对改善腰痛的作用。方法 选取2019年7月-2020年3月我院收治的90例慢性腰痛患者作为研究对象,根据椎旁肌肉脂肪浸润不同程度分为1级组(n=26)、2级组(n=36)和3级组(n=28),嘱患者做腰背肌肉锻炼3个月,比较各组初次就诊时及锻炼后3个月视觉模拟量表(VAS)评分和功能障碍指数问卷表(ODI)评分。结果 三组初次就诊时、锻炼3个月后VAS评分和ODI评分比较,差异有统计学意义(P<0.05);1级组锻炼3个月后VAS评分、ODI评分与初次就诊时比较,差异无统计学差异(P>0.05);2级组和3级组锻炼3个月后VAS评分、ODI评分低于初次就诊时,差异有统计学意义(P<0.05)。结论 脂肪浸润程度分级与腰痛评分有关,进行腰背部肌肉锻炼可以在一定程度上缓解肌肉退变引起的腰痛症状。
Abstract:
Objective To investigate the correlation between lumbar paravertebral muscle fat grading and low back pain score, and the effect of back muscle exercise on improving low back pain.Methods A total of 90 patients with chronic low back pain admitted to our hospital from July 2019 to March 2020 were collected. According to the different degrees of fat infiltration of paravertebral muscles, they were divided into level 1 group (n=26), level 2 group (n=36) and level 3 group (n=28).Patients were instructed to do low back muscle exercises for 3 months. The visual analog scale (VAS) scores and the oswestry disability index (ODI) questionnaire scores at the first visit and 3 months after exercise were compared in each group.Results There was significant difference in VAS score and ODI score at the first visit and 3 months after exercise among the three groups (P<0.05). The VAS score and ODI score of the level 1 group after 3 months of exercise were compared with those at the first visit, the difference was not statistically significant (P>0.05); the VAS scores and ODI scores after 3 months of exercise in the level 2 group and the level 3 group were lower than those at the first visit, the difference was statistically significant (P<0.05).Conclusion The degree of fat infiltration is related to the low back pain score, and low back muscle exercise can relieve the symptoms of low back pain caused by muscle degeneration to a certain extent.
