摘要:
目的 比较锁定钢板内固定与普通钢板内固定治疗复杂桡骨远端骨折的临床疗效。方法 选取2017年5月-2021年1月简阳市人民医院收治的89例复杂桡骨远端骨折患者作为研究对象,根据内固定材料不同分为对照组43例与研究组46例。对照组采用经掌侧入路普通钢板内固定治疗,研究组采用经掌侧入路锁定钢板内固定治疗,比较两组手术相关指标、腕关节功能恢复效果、影像学参数及并发症发生情况。结果 两组手术时间、术中出血量及住院时间比较,差异无统计学意义(P>0.05);研究组骨折愈合时间短于对照组,差异有统计学意义(P<0.05);研究组腕关节功能恢复优良率为93.48%,高于对照组的81.40%,差异有统计学意义(P<0.05);研究组术后掌倾角、尺偏角、桡骨高度大于对照组,桡腕关节面平整度低于对照组,差异有统计学意义(P<0.05);研究组术后并发症发生率为8.70%,低于对照组的20.93%,差异有统计学意义(P<0.05)。结论 锁定钢板内固定治疗复杂桡骨远端骨折较普通钢板内固定治疗的骨折愈合时间更短,更能改善腕关节功能和尺偏角、掌倾角以及桡骨远端高度,更能防止骨折复位丢失,且可降低并发症发生率,临床治疗效果更佳,功能恢复更好。
Abstract:
Objective To compare the clinical effects of locking plate internal fixation and common plate internal fixation in the treatment of complex distal radius fractures.Methods A total of 89 patients with complex distal radius fracture admitted to the People,s Hospital of Jianyang City from May 2017 to January 2021 were selected as the study subjects. According to different internal fixation materials, they were divided into control group(n=43)and study group(n=46). The control group was treated with ordinary plate internal fixation through volar approach, while the research group was treated with locked plate internal fixation through volar approach. The operation-related indexes, wrist function recovery effect, imaging parameters and complications of the two groups were compared.Results There were no significant differences in operation time, intraoperative blood loss and hospital stay between the two groups (P>0.05); the fracture healing time in the study group was shorter than that in the control group, and the difference was statistically significant (P<0.05). The excellent and good rate of wrist function recovery in the study group was 93.48%, which was higher than 81.40% in the control group, and the difference was statistically significant (P<0.05). The palmaric angle, ulnar deflection angle and radius height in the study group were higher than those in the control group, and the radiocarpal articular plane flatness was lower than that in the control group, the difference was statistically significant (P<0.05). The incidence of postoperative complications in the study group was 8.70%, which was lower than 20.93% in the control group, and the difference was statistically significant (P<0.05).Conclusion The fracture healing time of locking plate fixation in the treatment of complex distal radius fractures is shorter than that of ordinary plate fixation, which can improve the wrist function, ulnar deviation angle, palm inclination angle and distal radius height, prevent the loss of fracture reduction, and reduce the incidence of complications. The clinical treatment effect is better, and the functional recovery is better.
