摘要:
目的 分析血清D-二聚体（D-D）水平与高血压脑出血（HICH）患者病情严重程度及术后预后的关系。方法 选取我院2019年1月-2021年6月共86例高血压脑出血患者作为研究组，另选取同期80例健康体检者作为对照组，分别检测两组血清D-D水平。比较两组术前血清D-D水平，研究组不同疾病严重程度、不同转归HICH患者术后1、3、7、14天血清D-D水平，分析血清D-D水平与GCS评分及GOS评分的相关性。结果 研究组术前D-D水平高于对照组，差异有统计学意义（P＜0.05）；轻型患者血清D-D水平低于中型与重型患者，中型患者D-D水平低于重型患者，差异有统计学意义（P＜0.05）；两组术后1天血清D-D水平比较，差异无统计学意义（P＞0.05）；预后良好患者术后3、7、14天血清D-D水平低于预后不良患者，差异有统计学意义（P＜0.05）；Pearson相关性分析结果显示，血清D-D水平与GCS评分、GOS评分均呈负相关（r=-0.959、-0.669，P＜0.05）。结论 血清D-二聚体水平与HICH患者疾病严重程度以及术后预后均有一定的相关性，临床中可将其作为评估疾病以及预测预后的指标之一。
Abstract:
Objective To analyze the relationship between serum D-dimer (D-D) level and the severity and postoperative prognosis of hypertensive intracerebral hemorrhage (HICH) patients.Methods A total of 86 patients with hypertensive cerebral hemorrhage in our hospital from January 2019 to June 2021 were selected as the study group, and 80 healthy people in the same period were selected as the control group. The serum D-D levels of the two groups were detected respectively. The preoperative serum D-D levels of the two groups were compared. The serum D-D levels of HICH patients with different disease severity and different outcomes were compared at 1,3,7 and 14 days after operation. The correlation between serum D-D levels and GCS score and GOS score was analyzed.Results The preoperative D-D level of the study group was higher than that of the control group, and the difference was statistically significant (P<0.05). The serum D-D level of mild patients was lower than that of moderate and severe patients, and the D-D level of moderate patients was lower than that of severe patients, the difference was statistically significant (P<0.05). There was no statistically significant difference in serum D-D level 1 day after operation between the two groups (P>0.05). The serum D-D level of patients with good prognosis was lower than that of patients with poor prognosis on days 3, 7 and 14 after operation, and the difference was statistically significant (P<0.05). Pearson correlation analysis showed that serum D-D level was negatively correlated with GCS score and GOS score (r=-0.959, -0.669, P<0.05).Conclusion Serum D-dimer level has a certain correlation with the severity of HICH patients and postoperative prognosis, which can be used as one of the indicators to evaluate the disease and predict the prognosis in clinic.
