摘要:
目的 探索产程中剖宫产时期的选择对产后盆腔器官功能的影响。方法 选取2018年7月-2020年7月我院90产妇例作为研究对象,其中未临产30例设为A组,临产早期(宫口开大<6 cm)30例设为B组,临产晚期(宫口开大≥6 cm)30例设为C组。比较三组产后6~8周盆腔器官功能检查结果、盆腔器官脱垂(POP)、压力性尿失禁(SUI)情况及盆底肌评分。结果 B组和C组静息、缩肛和Valsalva动作状态下肛裂孔前后径、横径、横截面积、膀胱尿道后角均低于A组,且B组低于C组(P<0.05);C组产后6~8周POP发生率、SUI发生率均低于A组与B组(P<0.05);C组盆底肌力评分与A、B组比较(P<0.05)。结论 剖宫产的时期选择会对盆腔器官功能产生影响,产程中较早剖宫产对盆腔器官功能影响较小。
Abstract:
Objective To explore the effect of cesarean section during labor on postpartum pelvic organ function.Methods A total of 90 parturients of our hospital from July 2018 to July 2020 were selected as the research objects. Among them, 30 cases without labor were set as group A, 30 cases in the early stage of labor (cervical dilatation <6 cm) were set as group B, and 30 cases in the late stage of labor (cervical dilatation ≥ 6 cm) were set as group C. The results of pelvic organ function examination, pelvic organ prolapse (POP), stress urinary incontinence (SUI) and pelvic floor muscle score were compared among the three groups at 6-8 weeks postpartum.Results The anteroposterior diameter, transverse diameter, cross-sectional area and posterior vesicourethral angle of anal fissure in group B and group C were lower than those in group A at rest, anal contraction and Valsalva action, and those in group B were lower than those in group C (P<0.05). The incidence of POP and SUI in group C was lower than that in group A and group B at 6-8 weeks postpartum(P<0.05). There was significant difference in pelvic floor muscle score between group C and groups A and B (P<0.05).Conclusion The choice of cesarean section has influence on pelvic organ function, and early cesarean section has little influence on pelvic organ function.
