摘要:
目的 研究不同磁共振成像(MRI)序列对膝关节前后交叉韧带损伤的诊断价值。方法 选取2019年3月-2021年9月经我院确诊的40例膝关节前后交叉韧带损伤患者作为研究对象,依次给予MRI二维序列(2D-MRI)及三维序列(3D-MRI)检查,比较2D-MRI与3D-MRI的图像质量、诊断效能,同时分析其与关节镜诊断结果的一致性。结果 3D-MRI的信噪比、图像质量评分均高于2D-MRI,显示完整度评分小于2D-MRI(P<0.05)。以关节镜检查结果为金标准,3D-MRI对膝关节前后交叉韧带损伤的诊断准确性为92.50%、敏感度为96.67%、特异性为80.00%,高于2D-MRI的77.50%、92.31%、50.00%(P<0.05)。经Kappa检验,2D-MRI与关节镜诊断结果具有中等一致性(Kappa值=0.587),3D-MRI与关节镜诊断结果具有较好一致性(Kappa值=0.829)。结论 3D-MRI序列对膝关节前后交叉韧带损伤的图像检查质量优于2D-MRI序列,其诊断效能更为理想,与关节镜诊断具有较高一致性。
Abstract:
Objective To study the diagnostic Value of different magnetic resonance imaging (MRI) sequences on anterior and posterior cruciate ligament injury of knee joint.Methods From March 2019 to September 2021, 40 patients with anterior and posterior cruciate ligament injury of knee joint diagnosed in our hospital were selected as the research objects. Two-dimensional MRI (2D-MRI) and three-dimensional MRI (3D-MRI) were given in turn. The image quality and diagnostic efficacy of 2D-MRI and 3D-MRI were compared, and the consistency with arthroscopic diagnosis was analyzed.Results The signal-to-noise ratio and image quality score of 3D-MRI were higher than those of 2D-MRI, and the display integrity score was lower than that of 2D-MRI (P<0.05). Taking the results of arthroscopy as the gold standard, the diagnostic accuracy, sensitivity and specificity of 3D-MRI for anterior and posterior cruciate ligament injury of knee joint were 92.50%, 96.67% and 80.00%, respectively, which were higher than 77.50%, 92.31% and 50.00% of 2D-MRI(P<0.05). By Kappa test, 2D-MRI and arthroscopic diagnosis results have moderate consistency (Kappa value=0.587), 3D-MRI and arthroscopic diagnosis results have good consistency (Kappa value=0.829).Conclusion The image quality of 3D-MRI sequence for anterior and posterior cruciate ligament injury of knee joint is better than that of 2D-MRI sequence, its diagnostic efficiency is more ideal, and has high consistency with arthroscopic diagnosis.
