摘要:
目的 研究多模态磁共振成像对乳腺腺病与非钙化型乳腺癌的诊断价值。方法 选取2022年3月-2023年3月我院诊治的32例乳腺腺病与48例非钙化型乳腺癌患者为研究对象,均进行多模态磁共振成像检查,观察乳腺腺病与非钙化型乳腺癌患者MRI表现、扩散加权成像(DWI)、动态增强磁共振成像(DCE-MRI)及DWI+DCE-MRI联合诊断效能(灵敏度、特异度、准确率、阳性预测值、阴性预测值),并分析不同诊断技术对乳腺腺病与非钙化型乳腺癌的诊断准确率与术后病理结果的一致性。结果 乳腺腺病与非钙化型乳腺癌患者非肿块样强化的各种强化形态、内部特点、TIC曲线、ADC值比较,差异有统计学意义(P<0.05),而两组肿块样强化比较,差异无统计学意义(P>0.05);DWI+DCE-MRI联合诊断灵敏度、阴性预测值均高于DWI诊断,特异度、阳性预测值均低于DWI诊断(P<0.05);DWI+DCE-MRI联合诊断灵敏度、特异度、准确率、阳性预测值、阴性预测值与DCE-MRI诊断比较,差异无统计学意义(P>0.05);DWI+DCE-MRI联合诊断对乳腺腺病与非钙化型乳腺癌诊断结果与术后病理结果具有高度一致性,且一致性高于DWI、DCE-MRI检查患者(P<0.05)。结论 多模态磁共振成像对乳腺腺病与非钙化型乳腺癌鉴别诊断具有较高的价值,可表现出较显著的MRI临床特点,且DWI+DCE-MRI联合可提高诊断灵敏度,一定程度降低漏诊、误诊,值得临床应用。
Abstract:
Objective To study the diagnostic value of multimodal magnetic resonance imaging in adenosis of breast and non-calcified breast cancer.Methods A total of 32 patients with adenosis of breast and 48 patients with non-calcified breast cancer diagnosed and treated in our hospital from March 2022 to March 2023 were selected as the research objects. All patients underwent multimodal magnetic resonance imaging. The MRI manifestations, diffusion-weighted imaging (DWI), dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and DWI+DCE-MRI combined diagnostic efficacy (sensitivity, specificity, accuracy, positive predictive value, negative predictive value) of patients with adenosis of breast and non-calcified breast cancer were observed, and the consistency between the diagnostic accuracy of different diagnostic techniques for adenosis of breast and non-calcified breast cancer and the postoperative pathological results was analyzed.Results There were statistically significant differences in various enhancement patterns, internal characteristics, TIC curves and ADC values of non-mass-like enhancement in patients with adenosis of breast and non-calcified breast cancer (P<0.05), there was no significant difference in mass-like enhancement between the two groups (P>0.05). The sensitivity and negative predictive value of DWI+DCE-MRI combined diagnosis were higher than those of DWI diagnosis, and the specificity and positive predictive value were lower than those of DWI diagnosis (P<0.05), there was no significant difference in sensitivity, specificity, accuracy, positive predictive value and negative predictive value between DWI+DCE-MRI combined diagnosis and DCE-MRI diagnosis (P>0.05). The diagnostic results of DWI+DCE-MRI combined diagnosis of adenosis of breast and non-calcified breast cancer were highly consistent with the postoperative pathological results, and the consistency was higher than that of DWI and DCE-MRI (P<0.05).Conclusion Multimodal magnetic resonance imaging has a high value in the differential diagnosis of adenosis of breast and non-calcified breast cancer, and can show significant MRI clinical features. The combination of DWI+DCE-MRI can improve the diagnostic sensitivity and reduce missed diagnosis and misdiagnosis to a certain extent, which is worthy of clinical application.
