摘要:
目的 探讨降钙素原（PCT）联合血清淀粉样蛋白A（SAA）在急性胆管炎患者诊疗中的临床价值。方法 纳入2016年1月~2018年11月我院收治的136例急性胆管炎患者为研究对象。根据病情分级，分为轻度组（n=45）、中度组（n=67）和重度组（n=24）。于治疗前后检测所有患者的血PCT和SSA浓度。观察三组患者入院时的血PCT和SAA水平，评价其与病情严重程度的相关性并观察治疗后PCT和SSA浓度的变化。结果 治疗前，中度组患者的PCT和SSA水平高于轻度组（P＜0.05），而重度组的PCT和SSA水平高于轻度组和中度组，差异具有统计学意义（P＜0.05）。双变量相关性分析显示，PCT和SSA水平均与患者的病情分级呈显著正相关（r=0.736，P＜0.001；r=0.852，P＜0.001）；三组患者应用抗生素治疗5 d后，PCT和SSA较治疗前下降（P＜0.05），而治疗10 d后，PCT和SSA的水平进一步下降，低于治疗前和治疗5 d时的浓度（P＜0.05）。结论 对于急性胆管炎患者，PCT和SSA水平与患者的病情严重程度显著相关，可用于判断患者的病情和评估治疗效果。
Abstract:
Abstract：Objective To investigate the clinical value of procalcitonin (PCT) combined with serum amyloid A (SAA) in the diagnosis and treatment of patients with acute cholangitis.Methods A total of 136 patients with acute cholangitis admitted to our hospital from January 2016 to November 2018 were enrolled. According to the disease classification, they were divided into mild group (n=45), moderate group (n=67) and severe group (n=24). Blood PCT and SSA concentrations were measured in all patients before and after treatment. Blood PCT and SAA levels were observed at admission time among three groups time among three groups,evaluated for their association with severity and observed the changes of PCT and SSA concentrations after treatment.Results Before treatment, the PCT and SSA levels were higher in the moderate group than that in the mild group (P<0.05), while the PCT and SSA levels in the severe group were higher than those in the mild and moderate groups (P<0.05).Bivariate correlation analysis showed that PCT and SSA levels were significantly positively correlated with the patient's disease grade (r=0.736,P<0.001;r=0.852,P<0.001);After 5 d of antibiotic treatment, PCT and SSA decreased after treatment(P<0.05),and after 10 d of treatment, the levels of PCT and SSA decreased further, lower than the concentration before treatment and 5 d after treatment (P<0.05).Conclusion For patients with acute cholangitis, PCT and SSA levels are significantly associated with the severity of the patient's condition and can be used to determine the patient's condition and assess treatment outcomes.
