目的 比较三款治疗髂静脉疾病的血管支架(Wallstent、Smart control及Venovo)的物理性能,为腔内治疗髂静脉压迫和狭窄类疾病支架的选择提供指导依据。方法 应用自制加压系统测量三款血管支架的抗压力及弯折应力,在数字显影造影(DSA)-3D成形下观测三款支架在受压及弯折时的形变情况,用Imaje软件进行形变面积及形变截面长径和短径的测量并比较。结果在支架抗压力方面,Venovo抗压能力较好,Smart control次之,Wallstent略弱,差异有统计学意义(P<0.017);在弯折应力方面,Venovo较好,Smart control其次,Wallsent略差;在弯折形变方面,当弯折成150°、120°时,Wallstent与Smart control比较,差异无统计学意义(P>0.017),弯折至90°时,Smart control形变度明显增大,而Venovo在抗压性、柔顺性及弯折形变度方面均优于其他两款支架,差异有统计学意义(P<0.017)。结论 不同的血管支架在力学性能上有很大差异,在选择支架治疗髂静脉压迫、狭窄这类疾病时应参考支架的不同力学特点,在弯曲角度不大时,Smart control和Wallstent弯折形变相近,均可使用,但抗压性能则Smart control抗压能力更佳,且柔顺性更好;而Venovo支架抗压性和柔顺性都较好,在静脉严重狭窄和静脉弯曲度大时均可使用。
Abstract:
Objective To compare the physical properties of three vascular stents (Wallstent, Smart control and Venovo) for the treatment of iliac vein diseases, and to provide guidance for the choice of stent for endovascular treatment of iliac vein compression and stenosis diseases. Methods The self-made compression system was used to measure the compression resistance and bending stress of three vascular stents. The digital angiography (DSA) -3D was used to observe the deformation of the three stents under compression and bending, and the deformation was performed using Imaje software. Measure and compare the major and minor diameters of the area and deformation section. Results In terms of stent pressure resistance, Venovo has better compression resistance, Smart control is second, and Wallstent is slightly weaker,the difference is statistically significant (P <0.017). In terms of compliance, Venovo is better, Smart control is second, and Wallsent is slightly worse; In terms of bending deformation, when bending to 150 ° and 120 °, there was no significant difference between Wallstent and Smart control (P>0.017). When bending to 90 °, the deformation of Smart control increased significantly, and Venovo is superior to the other two stents in compression resistance, flexibility, and bending deformation,the differences are statistically significant (P<0.017). Conclusion The mechanical properties of different vascular stents are very different. When selecting a stent to treat diseases such as iliac vein compression and stenosis, you should refer to the different mechanical characteristics of the stent. When the bending angle is not large, the Smart control and Wallstent bend are similar. Both can be used, but the compression performance of Smart control is better, and the flexibility is better; the Venovo stent has better compression resistance and flexibility, and can be used when the vein is severely narrowed and the vein is curved.
