摘要:
目的 探讨初产妇经阴道分娩与剖宫产对产后压力性尿失禁（SUI）发生情况及盆底肌力的影响。方法 选择2019年1月~2020年1月在我院住院分娩的初产妇320例，依据分娩方式分为剖宫产组153例，阴道分娩组167例。比较两组产后SUI发生情况、产后不同时间（产后4周及8周）尿道旋转角度、膀胱颈活动度（UVJ-M）及盆底肌图值的活力值。结果 剖宫产组产后SUI发生率为9.80%，低于阴道分娩组的19.76%，差异有统计学意义（P＜0.05）；产后4周及8周阴道分娩组尿道旋转角度及UVJ-M大于剖宫产组，差异有统计学意义（P＜0.05）；阴道分娩组左右两侧活力值低于剖宫产组，差值高于剖宫产组，差异有统计学意义（P＜0.05）。结论 经阴道分娩的初产妇产后SUI发生率高于剖宫产，且对盆底肌功能影响较大，应注重阴道分娩产妇产后康复。
Abstract:
Objective To explore the effects of vaginal delivery and cesarean section of primipara on the occurrence of postpartum stress urinary incontinence (SUI) and pelvic floor muscle strength.Methods A total of 320 primiparas who were hospitalized in our hospital for delivery from January 2019 to January 2020 were selected. According to the delivery method, they were divided into 153 cases of cesarean section and 167 cases of vaginal delivery. The occurrence of postpartum SUI, urethral rotation angle, bladder neck mobility (UVJ-M) and pelvic floor muscles at different times (4 and 8 weeks postpartum) were compared between the two groups.Results The incidence of postpartum SUI in the cesarean section was 9.80%, which was lower than 19.76% in the vaginal delivery group,the difference was statistically significant (P＜0.05);The urethral rotation angle and UVJ-M of the vaginal delivery group at 4 weeks and 8 weeks postpartum were greater than those of the cesarean section group, the difference was statistically significant (P＜0.05);The vitality value of the left and right sides of the vaginal delivery group was lower than that of the cesarean section group, and the difference was higher than that of the cesarean section group, the difference was statistically significant (P＜0.05).Conclusion The incidence of postpartum SUI in primiparas with vaginal delivery was higher than that of cesarean section, and it had a greater impact on pelvic floor muscle function. The postpartum rehabilitation of vaginal delivery women should be emphasized.
