目的 探讨开放式胫骨高位截骨术治疗膝关节内侧间室骨关节炎合并膝内翻的早期临床效果。方法 选取2018年10月~2019年10月安徽医科大学附属省立医院收治的膝关节内侧间室骨关节炎合并膝内翻患者40例(40膝),均行开放式胫骨高位截骨术,比较术前及术后Lysholm评分、Tenger评分、股胫角(FTA)和胫骨近端内侧角(MPTA)变化及并发症发生情况。结果 40例患者均获得随访,时间10~22个月,平均随访时间为(13.17±2.25)个月。所有患者截骨愈合良好,均在3个月左右获得骨性愈合,未出现延迟愈合或不愈合。末次随访时Lysholm评分、Tenger评分高于术前,FTA小于术前,MPTA大于术前,差异有统计学意义(P<0.05)。9例患者出现对侧合页骨折、下肢深静脉血栓等并发症,并发症发生率为22.50%,经相应处理后均痊愈。结论 开放式胫骨高位截骨术治疗膝关节内侧间室骨关节炎合并膝内翻的早期临床效果良好,可提高患者膝关节功能,且可纠正内翻畸形状况。
Abstract:
Objective To explore the early clinical effects of open high tibial osteotomy in the treatment of osteoarthritis of the medial compartment of the knee combined with knee varus.Methods From October 2018 to October 2019, 40 patients (40 knees) with medial compartment osteoarthritis of the knee joint and knee varus who were admitted to the Provincial Hospital Affiliated to Anhui Medical University were selected. All patients underwent open high tibial osteotomy. The Lysholm score, Tenger score, femoral tibial angle (FTA) and proximal medial angle of tibia (MPTA) changes and complications were compared before and after surgery.Results All 40 patients were followed up for 10-22 months. The average follow-up time was (13.17±2.25) months. All patients healed well by osteotomy and achieved bone union in about 3 months without delayed union or nonunion. At the last follow-up, Lysholm score and Tenger score were higher than before operation, FTA was lower than before operation, and MPTA was higher than before operation, the difference was statistically significant (P<0.05).9 patients had complications such as contralateral hinge fractures and deep vein thrombosis of the lower extremities. The complication rate was 22.50%, and all were cured after corresponding treatment.Conclusion The open high tibial osteotomy has a good early clinical effect for the treatment of medial compartment osteoarthritis of the knee combined with knee varus. It can improve the knee joint function and correct the varus deformity.
