摘要:
目的 探讨输尿管软镜碎石术（RIRS）治疗肾下盏结石的效果，观察治疗后患者肾功能变化情况。方法 选取2018年1月~2020年12月在我院治疗的肾下盏结石患者83例作为研究对象，根据治疗方案分为观察组（43例）和对照组（40例），对照组给予经皮肾镜取石术（PCNL）治疗，观察组采用RIRS治疗，比较两组手术时间、术中出血量、住院时间、结石清除率、肾功能（血清Scr、BUN、GFR）变化及并发症发生情况。结果 观察组手术时间长于对照组，术中出血量和住院时间少于对照组，差异均有统计学意义（P＜0.05）；观察组结石清除率为83.72%，低于对照组的92.50%，但差异无统计学意义（P＞0.05）；术后两组血清Scr、BUN和GFR水平均较术前改善，差异有统计学意义（P＜0.05）；但两组血清Scr、BUN和GFR水平比较，差异无统计学意义（P＞0.05）；两组术后并发症发生率比较，差异无统计学意义（P＞0.05）。结论 与经皮肾镜取石术相比，输尿管软镜碎石术治疗肾下盏结石具有创伤小，出血少、恢复快等优点，且两种手术的结石清除率、并发症发生率基本一致，对患者肾功能无影响，安全性良好。
Abstract:
Objective To explore the effect of flexible ureteroscope lithotripsy (RIRS) in the treatment of calculus of lower kidney, and observe the changes in renal function of patients after treatment.Methods A total of 83 patients with inferior calyx stones treated in our hospital from January 2018 to December 2020 were selected as the research objects. According to the treatment plan, they were divided into observation group (43 cases) and control group (40 cases). Skin nephrolithotomy (PCNL) treatment, the observation group was treated with RIRS,the operation time, intraoperative blood loss, hospitalization time, stone clearance rate, renal function (serum Scr, BUN, GFR) changes and complications were compared between the two groups.Results The operation time of the observation group was longer than that of the control group, and the amount of blood loss and hospital stay were less than those of the control group,the differences were statistically significant (P＜0.05);The stone clearance rate in the observation group was 83.72%, which was lower than 92.50% in the control group, but the difference was not statistically significant (P>0.05);The levels of serum Scr, BUN and GFR in the two groups after operation were improved compared with that before operation,the difference was statistically significant(P＜0.05);However, there was no significant difference in serum Scr, BUN and GFR levels between the two groups (P>0.05);There was no significant difference in the incidence of postoperative complications between the two groups (P>0.05).Conclusion Compared with percutaneous nephrolithotomy, flexible ureteroscope lithotripsy has the advantages of less trauma, less bleeding, and faster recovery. The stone removal rate and complication rate of the two operations are basically the same. It has no effect on the patient’s renal function and has good safety.
