摘要:
目的 探究淋巴细胞亚群和免疫球蛋白水平与系统性红斑狼疮（SLE）合并感染的相关性及危险因素。方法 收集安徽医科大学第二附属医院2015年6月-2021年1月收治的193例SLE患者（共住院434例次），依据是否存在感染分为感染组150例次和非感染组283例次。收集患者临床资料，包括年龄、性别、狼疮活动度评分、T细胞亚群、免疫球蛋白 G（IgG）、补体C3、补体C4、中性粒细胞绝对值、淋巴细胞绝对值、CRP水平，感染组记录感染部位、病原体类型并比较。结果 两组性别、初诊/复诊、SLE病程持续时间、SLEDAL评分比较，差异无统计学意义（P＞0.05）；感染组主要感染部位前3位依次为肺部52例次（34.70%）、上呼吸道38例次（25.30%）、泌尿系35例次（23.30%），其中 17例次（11.30%）为多部位感染；感染组CD4+T细胞、CD8+T细胞、总T细胞、B淋巴细胞、淋巴细胞水平低于非感染组，年龄、中性粒细胞、CRP水平高于非感染组，差异有统计学意义（P＜0.05）；两组CD4+/CD8+比值、NK细胞水平、补体C3、C4、IgG水平比较，差异无统计学意义（P＞0.05）；CD4+T细胞绝对值、CD8+T细胞绝对值水平的减少是狼疮发生感染的危险因素。结论 淋巴细胞亚群的变化与狼疮发生感染有关，CD4+T细胞绝对值、CD8+T细胞绝对值减少时，感染的风险增加。
Abstract:
Objective To investigate the correlation and risk factors of lymphocyte subsets and immunoglobulin levels with systemic lupus erythematosus (SLE) complicated with infection.Methods A total of 193 SLE patients (434 hospitalized cases) admitted to the Second Affiliated Hospital of Anhui Medical University from June 2015 to January 2021 were enrolled. According to whether there was infection, they were divided into infection group (150 cases) and non-infection group (283 cases). The clinical data of patients were collected, including age, gender, lupus activity score, T cell subsets, immunoglobulin G (IgG), complement C3, complement C4, absolute neutrophil count, absolute lymphocyte count and CRP level. The infection site and pathogen type were recorded and compared in the infection group.Results There was no significant difference in gender, initial/reexamination, duration of SLE and SLEDAL score between the two groups (P>0.05). The top three main infection sites in the infection group were 52 cases (34.70%) of lung, 38 cases (25.30%) of upper respiratory tract and 35 cases (23.30%) of urinary system, of which 17 cases (11.30%) were multi-site infection. The levels of CD4+T cells, CD8+T cells, total T cells, B lymphocytes and lymphocytes in the infection group were lower than those in the non-infection group, and the age, neutrophil and CRP levels were higher than those in the non-infection group, the difference was statistically significant (P<0.05). There was no significant difference in CD4+/CD8+ ratio, NK cell level, complement C3, C4 and IgG levels between the two groups (P>0.05). The decrease in absolute values of CD4+T cells and CD8+ T cells was a risk factor for lupus infection.Conclusion The changes of lymphocyte subsets are related to lupus infection. When the absolute values of CD4+T cells and CD8+T cells decrease, the risk of infection increases.
