目的 分析引起肥胖儿童骨龄提前的可能因素,寻找骨龄提前的独立预测因素。方法 选择2013年8月-2020年8月就诊于我科的142例单纯性肥胖儿童作为研究对象,准确测量其身高、体重,采集空腹血糖、胰岛素、雌二醇(E2)、黄体生成素(LH)、卵泡刺激素(FSH)、睾酮(T)、硫酸脱氢表雄酮(DHEAS)及骨龄片,分析肥胖患儿骨龄-年龄(BA-CA)与性激素、DHEAS及胰岛素分泌指标的相关性,多因素回归分析骨龄提前的独立预测因素。结果 儿童平均年龄(10.45±2.22)岁,骨龄(G-P法)(11.85±2.33)岁,BMI(26.3±4.42)kg/m2,DHEAS(150.16±101.50)μg/dl;相关性分析显示,DHEAS标准差积分(SDS)与BA-CA呈正相关(女性:R2=0.32,男性:R2=0.47,P均<0.05);BMI SDS与BA-CA在整个队列(R2=0.41,P<0.05)及男性中(R2=0.48,P<0.05)呈正相关,在女性中无相关性(R2=0.13,P>0.05);HOMA-IR与BA-CA无相关性(P>0.05);多元线性回归分析显示,DHEAS SDS、BMI SDS均与BA-CA呈正相关。结论 DHEAS、BMI的增加是肥胖儿童骨龄进展的独立预测因素。
Abstract:
Objective To analyze the possible factors of early bone age in obese children, and to find the independent predictors of early bone age.Methods A total of 142 children with simple obesity who were treated in our department from August 2013 to August 2020 were selected as the research objects. Their height and weight were accurately measured. Fasting blood glucose, insulin, estradiol (E2), luteinizing hormone (LH), follicle stimulating hormone (FSH), testosterone (T), dehydroepiandrosterone sulfate (DHEAS) and bone age tablets were collected. The correlation between bone age-age (BA-CA) and sex hormones, DHEAS and insulin secretion indexes in obese children was analyzed. Multivariate regression analysis was used to analyze the independent predictors of advanced bone age.Results The average age of children was (10.45±2.22) years old, bone age was (11.85±2.33) years old, average BMI was (26.3±4.42) kg/m2, and DHEAS was (150.16±101.50) μg/dl; correlation analysis showed that DHEAS standard deviation score (SDS) was positively correlated with BA-CA (female:R2=0.32, male:R2=0.47, P<0.05); BMI SDS was positively correlated with BA-CA in the whole cohort (R2=0.41, P<0.05) and in males (R2=0.48, P<0.05), but not in females (R2=0.13, P>0.05); HOMA-IR was not correlated with BA-CA (P>0.05). Multivariate linear regression analysis showed that DHEAS SDS and BMI SDS were positively correlated with BA-CA.Conclusion Increased DHEAS and BMI are independent predictors of bone age progression in obese children.
