目的 研究急性ST段抬高型心肌梗死(STEMI)患者miR-199a-5p/沉默信息调节因子相关酶1(Sirt1)的变化及临床意义。方法 选择2020年12月-2021年6月在佳木斯大学临床医学院接受PCI治疗的60例STEMI患者作为STEMI组,同期在我院体检的60例健康志愿者作为对照组。比较两组外周血miR-199a-5p、Sirt1的表达水平,观察STEMI患者PCI后6个月主要不良心血管事件(MACE)发生情况。结果 STEMI组患者外周血miR-199a-5p的表达水平高于对照组、Sirt1的表达水平低于对照组,差异有统计学意义(P<0.05);与STEMI组中单支病变、Gensini积分0~40分、未发生MACE患者比较,多支病变、Gensini积分>40分、发生MACE患者的外周血miR-199a-5p的表达水平增加、Sirt1表达水平降低,差异有统计学意义(P<0.05);经Pearson检验,STEMI组患者外周血miR-199a-5p表达水平与外周血中Sirt1表达水平负相关(r=-0.519,P<0.05);经ROC曲线分析,外周血miR-199a-5p、Sirt1表达水平对STEMI患者PCI后发生MACE具有预测价值。结论 STEMI患者外周血miR-199a-5p表达增加及Sirt1表达降低存在相关性,并且与病情加重、预后不良有关。
Abstract:
Objective To study the changes and clinical significance of miR-199a-5p/silent information regulator transcript-1 (Sirt1) axis in patients with acute ST segment elevation myocardial infarction (STEMI).Methods A total of 60 STEMI patients who received PCI treatment in our hospital from December 2020 to June 2021 were selected as STEMI group, and 60 healthy volunteers who received physical examination in our hospital during the same period were selected as control group. The expression levels of miR-199a-5p and Sirt1 in peripheral blood were compared between the two groups, and the occurrence of major adverse cardiovascular events (MACE) in STEMI patients 6 months after PCI was observed.Results The expression level of mir-199a-5p in peripheral blood of STEMI group was higher than that of control group, and the expression level of Sirt1 was lower than that of control group, the difference was statistically significant (P<0.05). Compared patients with single vessel disease, Gensini score 0-40 and no MACE in STEMI group, the expression level of miR-199a-5p in peripheral blood of patients with multi vessel disease, Gensini score>40 and MACE increased and the expression level of Sirt1 decreased, the difference was statistically significant (P<0.05). Pearson test showed that there was a negative correlation between the expression level of miR-199a-5p in peripheral blood and the expression level of Sirt1 in peripheral blood in STEMI group (r=-0.519, P<0.05). ROC curve analysis showed that the expression levels of miR-199a-5p and Sirt1 in peripheral blood could predict MACE in STEMI patients after PCI.Conclusion There is correlation between the increased expression of miR-199a-5p and the decreased expression of Sirt1 in peripheral blood of STEMI patients, and also relates with the aggravation of the disease and poor prognosis.
