摘要:
目的 探讨幽门螺杆菌感染与结直肠腺瘤的关系。方法 选取2018年1月-2019年12月我院行13C尿素呼气试验及结肠镜检查的患者272例,将病理证实为结直肠腺瘤者作为结直肠腺瘤组,结肠镜检查正常者150例作为对照组。分析结直肠腺瘤及高危腺瘤的影响因素、结直肠腺瘤临床特征与Hp感染的关系,并采用非条件Logistic回归分析结直肠高危腺瘤的相关危险因素。结果 单因素分析显示,对照组与结直肠腺瘤组年龄、性别、Hp感染比较,差异有统计学意义(P<0.05);非高危腺瘤与高危腺瘤性别、Hp感染比较,差异有统计学意义(P<0.05);结肠腺瘤数量及腺瘤大小均与Hp感染相关(P<0.05),而腺瘤位置与Hp感染无关(P>0.05);非条件Logistic回归分析显示,男性及Hp感染是结直肠高危腺瘤的独立危险因素(P<0.05)。结论 性别及Hp感染与结直肠高危腺瘤密切相关,临床应对此予以重视。
Abstract:
Objective To investigate the relationship between Helicobacter pylori infection and colorectal adenoma.Methods A total of 272 patients who underwent 13C urea breath test and colonoscopy in our hospital from January 2018 to December 2019 were selected. Patients with colorectal adenoma confirmed by pathology were selected as colorectal adenoma group, and 150 cases with normal colonoscopy were selected as control group. The influencing factors of colorectal adenoma and high-risk adenoma, the relationship between clinical characteristics of colorectal adenoma and Hp infection were analyzed. The risk factors of high-risk adenoma were analyzed by non-conditional logistic regression.Results Univariate analysis showed that there were significant differences in age, gender and Hp infection between the control group and the colorectal adenoma group (P<0.05). There were significant differences in gender and Hp infection between non-high-risk adenomas and high-risk adenomas (P<0.05). The number and size of colonic adenomas were associated with Hp infection (P<0.05), while the location of adenomas was not associated with Hp infection (P>0.05). Unconditional Logistic regression analysis showed that male and Hp infection were independent risk factors for high-risk colorectal adenomas (P<0.05).Conclusion Gender and Hp infection are closely related to high-risk colorectal adenomas, which should be paid attention to clinically.
