摘要:
目的 研究LTA4H基因多态性对汉族儿童支气管哮喘患者使用孟鲁司特钠疗效及肺功能的影响。方法 收集厦门市中医院2019年10月-2021年3月就诊的70例经孟鲁司特钠治疗的哮喘患儿临床资料,均进行基因检测及肺功能检测,分析患儿LTA4H基因rs2660845位点基因型分布情况,比较不同基因型患儿临床疗效及各基因型患儿治疗前与治疗6周后肺功能。结果 经统计,70例患儿AA型16例(22.86%)、AG型25例(35.71%)、GG型29例(41.43%),等位基因频率为A 41.00%、G 59.00%;70例哮喘患儿经孟鲁司特钠治疗6周后总有效率为78.57%,AG型、GG型总有效率均高于AA型(P<0.05);AA型治疗前与治疗6周后FEV1、PEF、FEV1%、FVC、FEF25、FEF50、FEF75比较,差异无统计学意义(P>0.05);AG型治疗6周后FEV1、PEF、FVC高于治疗前(P<0.05),但治疗前后FEV1%、FEF25、FEF50、FEF75比较,差异无统计学意义(P>0.05);GG型治疗6周后FEV1、PEF、FEF25、FEF50、FEF75高于治疗前(P<0.05),但治疗前后FVC、FEV1%比较,差异无统计学意义(P>0.05)。结论 孟鲁司特钠对AA型患儿的疗效欠佳,推测AA基因型可能是孟鲁司特钠的不敏感基因型或可称为耐药基因型。孟鲁司特钠对AG型、GG型患儿的治疗效果较好,疗程结束后患儿肺功能较治疗前改善,提示等位基因G可能是孟鲁司特钠的敏感基因。
Abstract:
Objective To study the effect of LTA4H gene polymorphism on the efficacy and lung function of montelukast sodium in Han children with bronchial asthma.Methods The clinical data of 70 children with asthma treated with montelukast sodium in Xiamen Hospital of Traditional Chinese Medicine from October 2019 to March 2021 were collected. Gene detection and pulmonary function test were performed to analyze the genotype distribution of LTA4H gene rs2660845 locus, and to compare the clinical efficacy of children with different genotypes and the pulmonary function of children with different genotypes before and 6 weeks after treatment.Results According to statistics, there were 16 cases (22.86%) of AA type, 25 cases (35.71%) of AG type and 29 cases (41.43%) of GG type. The allele frequency was A 41.00% and G 59.00%. The total effective rate of 70 children with asthma after 6 weeks of treatment with montelukast sodium was 78.57%, and the total effective rate of AG type and GG type was higher than that of AA type (P<0.05). There was no significant difference in FEV1, PEF, FEV1%, FVC, FEF25, FEF50 and FEF75 between AA type before treatment and 6 weeks after treatment (P>0.05). After 6 weeks of AG treatment, FEV1, PEF and FVC were higher than those before treatment (P<0.05), however, there was no significant difference in FEV1%, FEF25, FEF50 and FEF75 before and after treatment (P>0.05). After 6 weeks of GG treatment, FEV1, PEF, FEF25, FEF50 and FEF75 were higher than before treatment (P<0.05), but there was no significant difference in FVC and FEV1% before and after treatment (P>0.05).Conclusion The efficacy of montelukast sodium in children with AA type is not good, it is speculated that AA genotype may be an insensitive genotype of montelukast sodium or a drug-resistant genotype. Montelukast sodium has a good therapeutic effect on children with AG and GG types. After the course of treatment, the lung function of children is improved compared with that before treatment, suggesting that allele G may be a sensitive gene of montelukast sodium.
