摘要:
目的 探讨腹腔镜下经腹腔腹膜前疝修补术（TAPP）与开放手术对腹股沟疝患者术后疼痛、切口满意度的影响。方法 选取2020年3月-2022年5月我院收治的腹股沟疝患者160例，按照随机数字表法分为对照组与观察组，每组80例。对照组接受开放手术治疗，观察组接受TAPP手术处理，比较两组围术期相关指标、术后不同时段疼痛状况[简化McGill疼痛问卷（SF-MPQ）]、切口满意度及应激反应指标变化。结果 观察组手术时间长于对照组，术中出血量、术后首次下床时间及住院时间少于对照组，差异有统计学意义（P＜0.05）；观察组术后各时段SF-MPQ评分均低于对照组，差异有统计学意义（P＜0.05）；观察组切口总满意度（93.75%）高于对照组（83.75%），差异有统计学意义（P＜0.05）；术后3天，观察组C反应蛋白（CRP）、皮质醇（Cor）及β-内啡肽（β-EP）水平均低于对照组，差异有统计学意义（P＜0.05）。结论 TAPP治疗腹股沟疝患者虽然较开放手术耗时长，但能够减少术中出血量，减轻术后疼痛，提高切口满意度，并减轻机体应激反应，更有助于患者术后恢复。
Abstract:
Objective To investigate the effect of transabdominal preperitoneal prosthesis (TAPP) and open surgery on postoperative pain and incision satisfaction of patients with inguinal hernia.Methods A total of 160 patients with inguinal hernia admitted to our hospital from March 2020 to May 2022 were selected and divided into control group and observation group according to the random number table method, with 80 patients in each group. The control group received open surgery, and the observation group received TAPP surgery. The perioperative related indicators, pain status at different time points after surgery [Short-form McGill Pain Questionnaire (SF-MPQ)], incision satisfaction and stress response indicators were compared between the two groups.Results The operation time of the observation group was longer than that of the control group, and the intraoperative blood loss, the first time to get out of bed after operation and the hospitalization time were less than those of the control group, the differences were statistically significant (P<0.05). The SF-MPQ scores of the observation group were lower than those of the control group at each time point after operation, and the difference was statistically significant (P<0.05). The total satisfaction of incision in the observation group (93.75%) was higher than that in the control group (83.75%), and the difference was statistically significant (P<0.05). At 3 days after operation, the levels of C-reactive protein (CRP), cortisol (Cor) and β-endorphin (β-EP) in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05).Conclusion Although the operation time of TAPP in the treatment of inguinal hernia is longer than that of open surgery, it can reduce intraoperative bleeding, reduce postoperative pain, improve incision satisfaction, and reduce the body’s stress response, which is more conducive to postoperative recovery.
