摘要:
目的 观察高血压患者Lp-PLA2、Hcy、CRP、Lp(a)、AT-Ⅲ联合预测颈动脉粥样硬化发生的风险。方法 选择2018年1月-2019年5月我院收治的原发性高血压患者,按照合并颈动脉粥样硬化情况将其分为两组,即高血压组193例,高血压合并颈动脉粥样硬化组192例,另设健康对照组151例。行颈动脉超声检查,检测颈动脉内膜中层厚度(CIMT)。比较三组各指标水平及与CIMT的相关性。应用ROC曲线分析单项及联合诊断颈动脉粥样硬化的价值。结果 高血压组Lp-PLA2、Hcy高于对照组,AT-Ⅲ低于对照组(P<0.05);高血压合并颈动脉粥样硬化组Lp-PLA2、Hcy、Lp(a)高于高血压组(P<0.05),高血压合并颈动脉粥样硬化组Lp-PLA2、Hcy、Lp(a)粥样硬化所占比高于高血压组(P<0.05);Lp-PLA2、Hcy、CRP、Lp(a)、AT-Ⅲ联合诊断曲线下面积(AUC)最大。结论 在高血压患者中,Lp-PLA2、Hcy、CRP、Lp(a)、AT-Ⅲ联合检测对于早期预测颈动脉粥样硬化有较大的价值,早于颈动脉超声检查。
Abstract:
Objective To observe the value of Lp-PLA2, Hcy, CRP, Lp(a) and AT-Ⅲ in predicting the risk of carotid atherosclerosis in patients with hypertension.Methods Patients with essential hypertension admitted in our hospital from January 2018 to May 2019 were selected and divided into two groups according to the combination of carotid atherosclerosis, namely the hypertension group (193 patients) and the hypertension combined with carotid atherosclerosis group (192 patients), and 151 healthy controls was set as healthy control group. All subjects underwent carotid ultrasonography, and carotid intima-media thickness (CIMT) was measured. The levels of each index in the three groups and their correlation with CIMT were compared. ROC curve was used to analyze the value of single and combined diagnosis of carotid atherosclerosis.Results Lp-PLA2 and Hcy in the hypertension group were higher than those in the control group, and AT-Ⅲ was lower than that in the control group (P<0.05). The levels of Lp-PLA2, Hcy and Lp(a) in the hypertension combined with carotid atherosclerosis group were higher than those in the hypertension group (P<0.05). The proportion of Lp-PLA2, Hcy and Lp(a) atherosclerosis in the hypertension combined with carotid atherosclerosis group was higher than that in the hypertension group (P<0.05). The area under the curve (AUC) of Lp-PLA2, Hcy, CRP, Lp(a) and AT-Ⅲ was the largest.Conclusion For patients with hypertension, the combined detection of Lp-PLA2, Hcy, CRP, Lp(a) and AT-Ⅲ has great value in early prediction of carotid atherosclerosis, which is better than carotid ultrasound.
