摘要:
目的 探讨宫腔镜电切术联合宫腔放置左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的效果。方法 选择2020年4月-2023年3月我院收治的子宫内膜息肉患者80例作为研究对象，按照随机数字表法分为对照组和研究组，各40例，对照组采取宫腔镜电切术治疗，研究组在对照组基础上联合宫腔放置左炔诺孕酮宫内缓释系统治疗，比较两组子宫内膜厚度、月经量、雌激素、孕激素受体表达水平、临床疗效及随访半年复发情况。结果 研究组治疗后子宫内膜厚度、月经量低于治疗前，且低于对照组（P＜0.05）；研究组治疗后腺皮质、间质的雌激素及孕激素受体表达水平低于治疗前，且低于对照组（P＜0.05）；研究组治疗总有效率为95.00%，高于对照组的72.50%（P＜0.05）；研究组随访半年复发率为2.50%，低于对照组的12.50%（P＜0.05）。结论 子宫内膜息肉患者采取宫腔镜电切术联合宫腔放置左炔诺孕酮宫内缓释系统治疗的效果良好，可缩小子宫内膜厚度，恢复正常月经情况，改善激素受体表达水平，降低复发率。
Abstract:
Objective To investigate the effect of hysteroscopic electrotomy combined with intrauterine placement of levonorgestrel-releasing intrauterine system in the treatment of endometrial polyps.Methods A total of 80 patients with endometrial polyps admitted to our hospital from April 2020 to March 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 40 patients in each group. The control group was treated with hysteroscopic electrotomy, and the study group was treated with levonorgestrel-releasing intrauterine system placed in the uterine cavity on the basis of the control group. The endometrial thickness, menstrual volume, estrogen and progesterone receptor expression levels, clinical efficacy and recurrence after half a year of follow-up were compared between the two groups.Results The endometrial thickness and menstrual volume of the study group after treatment were lower than those before treatment, and lower than those of the control group (P<0.05). After treatment, the expression levels of estrogen and progesterone receptors in the glandular cortex and stroma of the study group were lower than those before treatment, and lower than those in the control group (P<0.05). The total effective rate of treatment in the study group was 95.00%, which was higher than 72.50% in the control group (P<0.05). The recurrence rate of the study group was 2.50%, which was lower than 12.50% of the control group (P<0.05).Conclusion Hysteroscopic electrotomy combined with intrauterine placement of levonorgestrel-releasing intrauterine system in patients with endometrial polyps has a good effect, which can reduce endometrial thickness, restore normal menstruation, improve hormone receptor expression level and reduce recurrence rate.
