摘要:
目的 探讨腹腔镜经腹腹膜前疝修补术治疗腹股沟疝的临床疗效及对其术后疼痛度的影响。方法 按照随机数字表法将南昌大学第一附属医院靖安医院2020年5月-2023年5月收治的80例腹股沟疝气患者分成对照组和研究组，各40例。对照组行开放式腹膜前间隙疝修补术，研究组行腹腔镜经腹腹膜前疝修补术。比较两组患者的术后疼痛程度、术后并发症发生率、手术指标，以及氧化应激水平。结果 研究组术后6、12、24、48、60 h疼痛度低于对照组（P＜0.05）。研究组术后并发症发生率低于对照组（P＜0.05）。研究组手术时间、离床活动时间、住院时间均低于对照组，术中出血量少于对照组，切口长度短于对照组（P＜0.05）。两组治疗后SOD、GSH-Px水平低于治疗前，MDA水平高于治疗前，但研究组SOD、GSH-Px水平高于对照组，MDA水平低于对照组（P＜0.05）。结论 腹腔镜经腹腹膜前疝修补术治疗可以降低腹股沟疝患者术后疼痛程度，改善患者氧化应激水平，加快患者术后康复速度，安全性较高，值得临床应用。
Abstract:
Objective To investigate the clinical efficacy of laparoscopic transabdominal preperitoneal hernia repair in the treatment of inguinal hernia and its effect on postoperative pain.Methods According to the random number table method, 80 patients with inguinal hernia admitted to Jing′an Hospital of the First Affiliated Hospital of Nanchang University from May 2020 to May 2023 were divided into control group and study group, with 40 patients in each group. The control group underwent open preperitoneal space hernia repair, and the study group underwent laparoscopic transabdominal preperitoneal hernia repair. The degree of postoperative pain, the incidence of postoperative complications, surgical indicators, and oxidative stress levels were compared between the two groups.Results The pain degree of the study group was lower than that of the control group at 6, 12, 24, 48 and 60 h after operation (P<0.05). The incidence of postoperative complications in the study group was lower than that in the control group (P<0.05). The operation time, ambulation time and hospitalization time of the study group were lower than those of the control group, the intraoperative blood loss was less than that of the control group, and the incision length was shorter than that of the control group (P<0.05). After treatment, the levels of SOD and GSH-Px in the two groups were lower than those before treatment, and the level of MDA was higher than that before treatment, but the levels of SOD and GSH-Px in the study group were higher than those in the control group, and the level of MDA was lower than that in the control group (P<0.05).Conclusion Laparoscopic transabdominal preperitoneal hernia repair can reduce the degree of postoperative pain in patients with inguinal hernia, improve the level of oxidative stress, and accelerate the recovery of patients after surgery. It is safe and worthy of clinical application.
