摘要:
目的 探讨腕踝针联合多模式镇痛在髋关节置换术后促进快速康复的效果。方法 选择2020年1月-2022年8月台州市中西医结合医院骨科收治的100例股骨颈骨折髋关节置换患者作为研究对象，以随机数字表法分为对照组与治疗组，每组50例。对照组实施单纯多模式镇痛，治疗组实施腕踝针联合多模式镇痛。比较两组疼痛情况（术后1、24、48、72 h VAS评分）、镇痛效果、髋关节功能（Harris）评分及不良反应（恶心呕吐、头晕、尿潴留）发生情况。结果 治疗组术后1、24、48、72 h VAS评分低于对照组（P＜0.05）；治疗组镇痛总有效率（98.00%）高于对照组（94.00%）（P＜0.05）；治疗组术后1、24、48、72 h Harris评分高于对照组（P＜0.05）；治疗组不良反应发生率（12.00%）低于对照组（48.00%）（P＜0.05）。结论 腕踝针联合多模式镇痛方案，可以提高围术期安全性，减轻术后疼痛，降低不良反应发生及加速术后康复，值得临床应用。
Abstract:
Objective To explore the effect of wrist-ankle acupuncture combined with multimodal analgesia in promoting rapid rehabilitation after hip replacement. Methods A total of 100 Patients with femoral neck fracture undergoing hip replacement in Department of Orthopedics, Taizhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to August 2022 were divided into control and treatment groups by random table method, with 50 patients in each group. The control group was treated with simple multimodal analgesia, and the treatment group was treated with wrist-ankle acupuncture combined with multimodal analgesia. The pain (VAS score at 1, 24, 48, 72 h after operation), analgesic effect, hip function (Harris) score and adverse reactions (nausea and vomiting, dizziness, urinary retention) were compared between the two groups. Results The VAS scores at 1, 24, 48 and 72 h after operation in the treatment group were lower than those in the control group (P<0.05). The total effective rate of analgesia in the treatment group (98.00%) was higher than that in the control group (94.00%) (P<0.05). The Harris score of the treatment group was higher than that of the control group at 1, 24, 48 and 72 h after operation (P<0.05). The incidence of adverse reactions in the treatment group (12.00%) was lower than that in the control group (48.00%)(P<0.05). Conclusion Wrist-ankle acupuncture combined with multimodal analgesia can improve perioperative safety, reduce postoperative pain, reduce adverse reactions and accelerate postoperative rehabilitation, which is worthy of clinical application.
