摘要:
目的 探究卵巢囊肿患者行腹腔镜卵巢囊肿剥除术对其临床指标、卵巢功能和性激素水平的影响。方法 共纳入60例卵巢囊肿患者，均于2021年1月-2024年10月在上饶市广信区人民医院接受诊治。采用随机数字表法将其分为两组：研究组与对照组，各30例。对照组接受腹腔镜单孔卵巢病损切除术治疗，研究组接受腹腔镜卵巢囊肿剥除术治疗。比较两组临床指标、卵巢功能[抗穆勒氏管激素（AMH）、卵泡计数（AFC）]、性激素水平[卵泡刺激素（FSH）、黄体生成素（LH）、雌二醇（E2）]、复发率。结果 与对照组相比，研究组临床指标更优，表现为手术时间、术后下床活动时间、住院时间更短，术中失血量更少（P＜0.05）；在卵巢功能方面，与术前相比，两组术后3个月AMH、AFC均降低，但与对照组相比，研究组AMH、AFC更高（P＜0.05）；在性激素水平方面，与术前相比，两组术后3个月FSH、LH水平均升高，而E2水平均降低，且与对照组相比，研究组FSH、LH水平更低，E2水平更高（P＜0.05）；研究组术后复发率（3.33%）低于对照组（26.67%）（P＜0.05）。结论 腹腔镜卵巢囊肿剥除术可改善卵巢囊肿患者临床指标，并在减少卵巢功能损伤及术后复发、维持性激素分泌水平方面具有积极作用。
Abstract:
Objective To investigate the effect of laparoscopic ovarian cystectomy on clinical indexes, ovarian function and sex hormone levels in patients with ovarian cyst. Methods A total of 60 patients with ovarian cysts were included, all of whom were treated in Shangrao Guangxin District People′s Hospital from January 2021 to October 2024. They were divided into two groups by random number table method: study group and control group, each group had 30 patients. The control group was treated with laparoscopic single-hole ovarian lesion resection, and the study group was treated with laparoscopic ovarian cystectomy. The clinical indexes, ovarian function [anti-Müllerian hormone (AMH), antral follicle count (AFC)], sex hormone levels [follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2)] and recurrence rate were compared between the two groups. Results Compared with the control group, the clinical indexes of the study group were better, showing shorter operation time, postoperative ambulation time, hospitalization time, and less intraoperative blood loss (P<0.05). In terms of ovarian function, compared with before operation, AMH and AFC in the two groups were decreased at 3 months after operation, but compared with the control group, AMH and AFC in the study group were higher (P<0.05). In terms of sex hormone levels, compared with before operation, the levels of FSH and LH in the two groups were increased, the level of E2 was decreased at 3 months after operation, while compared with the control group, the levels of FSH and LH in the study group were lower, and the level of E2 was higher (P<0.05). The recurrence rate of the study group (3.33%) was lower than that of the control group (26.67%) (P<0.05). Conclusion Laparoscopic ovarian cystectomy can improve the clinical indexes of patients with ovarian cysts, and has a positive effect on reducing ovarian function damage and postoperative recurrence, and maintaining sex hormone secretion levels.
