摘要:
目的 对比内镜下高频电切术与内镜下黏膜切除术（EMR）治疗胃息肉的临床效果。方法 采用随机对照研究设计，以2024年1月-12月万安县人民医院收治的60例胃息肉患者为对象，按照随机数字表法均分为两组，即对照组30例、研究组30例。对照组行内镜下高频电切术治疗，研究组则采用EMR治疗，比较两组手术相关指标（手术时间、出血量、住院时间）、临床疗效、炎症因子水平[白细胞计数（WBC）、C反应蛋白（CRP）]、并发症发生率与复发率。结果 研究组手术时间与住院时间均较对照组更短，且出血量较对照组更少（P＜0.05）；研究组治疗总有效率（96.67%）高于对照组（83.33%）（P＜0.05）；两组术后WBC、CRP水平均较术前升高，但与对照组术后炎症因子水平比较，研究组WBC、CRP水平更低（P＜0.05）；研究组并发症发生率与复发率均低于对照组（P＜0.05）。结论 EMR治疗胃息肉效果肯定，其息肉切除彻底、手术时间短、出血量少、术后恢复快、并发症少，可改善术后炎症状态，降低病灶复发风险，整体疗效优于内镜下高频电切术。
Abstract:
Objective To compare the clinical effects of endoscopic high-frequency electrotomy and endoscopic mucosal resection (EMR) in the treatment of gastric polyps. Methods Adopting a randomized controlled study design, 60 patients with gastric polyps admitted to Wan′an County People′s Hospital from January to December 2024 were enrolled as study subjects. They were equally divided into two groups using a random number table: a control group (30 patients) and a study group (30 patients). The control group was treated with endoscopic high-frequency electrotomy, while the study group was treated with EMR. The operation-related indicators, clinical efficacy, inflammatory factor levels, complication rate and recurrence rate were compared between the two groups. Results The study group demonstrated significantly shorter operation time and hospitalization time compared to the control group, exhibiting a marked reduction in bleeding volume as well (P<0.05). A significantly higher total treatment effective rate was observed in the study group (96.67%) compared to the control group (83.33%) (P<0.05). In both groups, white blood cell count (WBC), C-reactive protein (CRP) levels were higher postoperatively than preoperatively; nevertheless, significantly lower levels of these inflammatory markers were observed in the study group relative to the control group (P<0.05). The study group demonstrated lower complication and recurrence rates compared to the control group (P<0.05). Conclusion EMR is effective in the treatment of gastric polyps, which has the advantages of complete resection of polyps, short operation time, less bleeding, rapid postoperative recovery and less complications. Meanwhile, it can improve the postoperative inflammatory state and reduce the risk of recurrence of lesions. The overall efficacy is better than that of endoscopic high-frequency electrotomy.
