摘要:
目的 探讨宫颈液基细胞学联合阴道镜检查在宫颈病变诊断中的应用。方法 选择2018年6月~2019年6月我院收治的316例疑似宫颈病变患者为研究对象，采用随机数字表法分为对照组和观察组，各158例。对照组采用阴道镜检查，观察组在阴道镜检查基础上联合宫颈液基细胞学检查，比较两组阳性检出率、病理组织学诊断准确率、宫颈上皮内癌变（CIN）各级检出率。结果 观察组阳性检出率（25.94%）与对照组（24.05%）比较，差异无统计学意义（P＞0.05）；观察组病理组织学诊断准确率为70.74%，高于对照组的31.57%，差异有统计学意义（P＜0.05）；观察组CIN分级检出率与对照组比较，差异无统计学意义（P＞0.05）。结论 在宫颈液基细胞学基础上联合阴道镜检查可提高宫颈疾病临床检查准确率，但二者联合检查对阳性率及分级检出率并无明显优势，临床可根据具体情况选择筛查方法，以提高诊断率，避免漏诊。
Abstract:
Objective To explore the application of cervical liquid-based cytology combined with colposcopy in the diagnosis of cervical lesions. Methods A total of 316 patients with suspected cervical lesions admitted to our hospital from June 2018 to June 2019 were selected as the research object. They were divided into a control group and an observation group with a random number table method, 158 cases in each. The control group used colposcopy, and the observation group combined with cervical liquid-based cytology on the basis of colposcopy, compared the positive detection rate, pathological diagnosis accuracy rate, and cervical intraepithelial carcinoma(CIN) detection rate of the two groups.Results Compared with the control group (24.05%), the positive detection rate of the observation group (25.94%) was not statistically significant (P＞0.05); the accuracy of histopathological diagnosis in the observation group was 70.74%, which was higher than 31.57% of the control group,the difference was statistically significant (P＜0.05). Compared with the control group, the detection rate of CIN classification in the observation group was not statistically significant (P＞0.05).Conclusion Combined with colposcopy on the basis of cervical fluid-based cytology can improve the accuracy of clinical examination of cervical diseases, but the combined examination of the two has no obvious advantage in positive rate and graded detection rate. The clinical screening method can be selected according to specific circumstances. To improve the diagnosis rate and avoid missed diagnosis.
