摘要:
目的 研究血清CRP与肿瘤标志物检测在消化道恶性肿瘤中的临床价值。方法 选取2020年6月-2021年6月在我院诊治的94例消化道肿瘤患者为研究对象，依据良恶性分为良性组（n=61）和恶性组（n=33），并选取40例同期在我院体检健康者为对照组，比较3组患者血清CRP和肿瘤标志物（CEA、AFP、CA125、CA19-9）水平、消化道肿瘤患者血清CRP和肿瘤标志物阳性检出率、血清CRP、肿瘤标志物以及血清CRP联合肿瘤标志物检测消化道恶性肿瘤的敏感度、特异度。结果 恶性组、良性组血清CRP、CEA、AFP、CA125、CA19-9均高于对照组，恶性组血清CRP低于良性组，AFP、CEA、CA125、CA19-9均高于良性组，差异有统计学意义（P＜0.05）；恶性组患者血清CRP阳性检出率低于良性组，CEA、AFP、CA125、CA19-9阳性检出率均高于良性组，差异有统计学意义（P＜0.05）；血清CRP联合肿瘤标志物检测诊断消化道恶性肿瘤敏感度均高于血清CRP、肿瘤标志物检测，特异度低于血清CRP、肿瘤标志物检测，差异有统计学意义（P＜0.05）。结论 消化道恶性肿瘤患者肿瘤标志物、血清CRP水平显著升高，可作为良恶性肿瘤鉴别诊断的重要指标，且血清CRP与肿瘤标志物联合检测诊断恶性肿瘤敏感度和特异度高，可为临床肿瘤治疗和预防复发提供一定的参考。
Abstract:
Objective To study the clinical value of serum CRP and tumor markers in gastrointestinal malignant tumors.Methods A total of 94 patients with digestive tract tumors who were treated in our hospital from June 2020 to June 2021 were selected as the research subjects. According to the benign and malignant, they were divided into benign group (n=61) and malignant group (n=33), and 40 healthy people who underwent physical examination in our hospital at the same time were selected as the control group. The serum CRP and tumor markers (CEA, AFP, CA125, CA19-9) levels, the positive detection rate of serum CRP and tumor markers in patients with digestive tract tumors, the sensitivity and specificity of serum CRP, tumor markers and serum CRP combined with tumor markers in the detection of digestive tract malignant tumors among the three groups were compared.Results The serum CRP, CEA, AFP, CA125 and CA19-9 in the malignant group and the benign group were higher than those in the control group, the serum CRP in the malignant group was lower than that in the benign group, and the serum AFP, CEA, CA125 and CA19-9 in the malignant group were higher than those in the benign group, the difference was statistically significant (P<0.05). The positive detection rate of serum CRP in the malignant group was lower than that in the benign group, and the positive detection rates of CEA, AFP, CA125 and CA19 - 9 in the malignant group were higher than those in the benign group, the difference was statistically significant(P<0.05). The sensitivity of serum CRP combined with tumor markers in the diagnosis of gastrointestinal malignant tumors was higher than that of serum CRP and tumor markers, and the specificity was lower than that of serum CRP and tumor markers, and the difference was statistically significant (P<0.05).Conclusion The levels of tumor markers and serum CRP in patients with digestive tract malignant tumors are significantly increased, which can be used as an important indicator for the differential diagnosis of benign and malignant tumors. The sensitivity and specificity of combined detection of serum CRP and tumor markers in the diagnosis of malignant tumors are high, which can provide certain reference for clinical tumor treatment and prevention of recurrence.
