摘要:
目的 探讨腹腔镜下卵巢子宫内膜样囊肿剥除术对患者肿瘤标志物及卵巢储备功能的影响。方法 选择2019年6月-2020年8月本院收治的88例子宫内膜样囊肿患者，根据随机数字表法分为研究组和对照组，各45例。比较两组术前、术后7天肿瘤标志物[神经元特异性烯醇化酶（NSE）、糖类抗原199（CA199）、癌胚抗原（CEA）]、卵巢储备功能[雌二醇（E2）、抗缪勒氏管激素（AMH）]及窦卵泡数（AFC）；两组手术时间、总出血量、术后引流时间、住院时间、术后并发症发生率；两组随访1年的术后妊娠率、疾病复发率。结果 两组术后7天NSE、CA199、CEA、E2均降低，且研究组低于对照组，差异有统计学意义（P＜0.05）；两组AMH、AFC均增高，且研究组高于对照组，差异有统计学意义（P＜0.05）；研究组手术时间、总出血量、术后引流时间、住院时间低于对照组，差异有统计学意义（P＜0.05）；研究组术后并发症发生率（4.55%）低于对照组（22.73%），差异有统计学意义（P＜0.05）；研究组术后1年妊娠率（56.82%）高于对照组（34.09%），差异无统计学意义（P＞0.05）；研究组疾病复发率（4.55%）低于对照组（9.09%），差异无统计学意义（P＞0.05）。结论 腹腔镜下行卵巢子宫内膜样囊肿剥除术，可降低肿瘤标志物水平，改善卵巢储备功能，还可减轻手术创伤，缩短康复进程，改善远期预后。
Abstract:
Objective To investigate the effect of laparoscopic ovarian endometrioid cystectomy on tumor markers and ovarian reserve function.Methods A total of 88 patients with endometrioid cysts admitted to our hospital from June 2019 to August 2020 were selected and divided into study group and control group according to random number table method, with 45 cases in each group. The tumor markers [neuron-specific enolase (NSE), carbohydrate antigen 199 (CA199), carcinoembryonic antigen (CEA)], ovarian reserve function [estradiol (E2), anti-Müllerian hormone (AMH)] and the number of antral follicles (AFC) were compared between the two groups before and 7 days after operation. The operation time, total bleeding volume, postoperative drainage time, hospitalization time and incidence of postoperative complications in the two groups; the pregnancy rate and disease recurrence rate of the two groups were followed up for 1 year.Results NSE, CA199, CEA and E2 in the two groups decreased at 7 days after operation, and those in the study group were lower than the control group, the difference was statistically significant (P<0.05). AMH and AFC in the two groups were increased, and those in the study group were higher than the control group, the difference was statistically significant (P<0.05). The operation time, total bleeding volume, postoperative drainage time and hospitalization time of the study group were lower than those of the control group, and the difference was statistically significant (P<0.05). The incidence of postoperative complications in the study group (4.55%) was lower than that in the control group (22.73%), and the difference was statistically significant (P<0.05). The pregnancy rate at 1 year after operation in the study group (56.82%) was higher than that in the control group (34.09%), and the difference was not statistically significant (P>0.05). The disease recurrence rate in the study group (4.55%) was lower than that in the control group (9.09%), and the difference was not statistically significant (P>0.05).Conclusion Laparoscopic ovarian endometrioid cystectomycan reduce the level of tumor markers, improve ovarian reserve function, reduce surgical trauma, shorten the rehabilitation process, and improve long-term prognosis.
