摘要:
目的 探讨血液滤过联合益生菌治疗重症急性胰腺炎(SAP)的临床效果。方法 选取2018年6月-2021年3月厦门新开元医院及厦门大学附属成功医院收治的78例SAP患者作为研究对象,按照随机数字表法分为对照组42例和联合组36例。对照组采用常规治疗及血液滤过,联合组在对照组基础上联合益生菌治疗,比较两组炎症因子(CRP、IL-10、TNF-α)、肠道黏膜损伤程度(内毒素、D-乳酸)、SAP并发症发生率、ICU住院时间及总住院时间。结果 联合组CRP、IL-10、TNF-α、内毒素及D-乳酸水平均低于对照组,差异有统计学意义(P<0.05);联合组SAP并发症发生率低于对照组,ICU住院时间及总住院时间短于对照组,差异有统计学意义(P<0.05)。结论 血液滤过联合益生菌能改善SAP炎症反应及肠道损伤程度,降低并发症发生率,缩短住院时间,临床应用效果良好。
Abstract:
Objective To investigate the clinical effect of hemofiltration combined with probiotics in the treatment of severe acute pancreatitis (SAP).Methods A total of 78 patients with SAP admitted to Xinkaiyuan Hospital and Chenggong Hospital,Xiamen University from June 2018 to March 2021 were selected as subjects. According to the random number table method, they were divided into control group (42 cases) and combined group (36 cases). The control group was treated with routine treatment and hemofiltration, and the combined group was treated with probiotics on the basis of the control group. The inflammatory factors (CRP, IL-10, TNF-α), intestinal mucosal injury (endotoxin, D-lactic acid), SAP complication rate, ICU hospitalization time and total hospitalization time were compared between the two groups.Results The levels of CRP, IL-10, TNF-α, endotoxin and D-lactic acid in the combined group were lower than those in the control group, and the difference was statistically significant (P<0.05). The incidence of SAP complications in the combined group was lower than that in the control group, and the ICU hospitalization time and total hospitalization time were shorter than those in the control group, the difference was statistically significant (P<0.05).Conclusion Hemofiltration combined with probiotics can improve the inflammatory response and intestinal injury of SAP, reduce the incidence of complications and shorten the hospitalization time. The clinical application effect is good.
